外耳道软骨尖与颞骨外段面神经干立体解剖定位及临床应用

摘　　要：目的探讨手术过程中外耳道软骨尖与颞骨外段面神经干立体解剖定位及手术应用。方法2016年9月~2020年9月,对行腮腺肿瘤切除术患者42例及第一鳃裂瘘管切除患者2例术中冠状位外耳道软骨尖与面神经主干相对位置进行测量。术中暴露外耳道软骨尖,解剖面神经主干后,冠状位测量颞骨外段面神经干与外耳道软骨尖立体毗邻关系。结果在外科手术中发现颞骨外段面神经干位置相对恒定,冠状位以外耳道软骨尖为圆心,冠状位上面神经干坐标(0.62~1.13cm,0.59~1.11cm)。结论在冠状位上,以外耳道软骨尖为圆心的颞骨外段面神经干的立体定位,对外科手术有指导意义。Objective Discuss the three-dimensional anatomy location of the tragal pointer and the facial nerve external temporal bone during the operation.Methods Forty-two cases excision of parotid tumor and two cases excision of first branchial Fistula(Right side 20 cases,Left side 24 cases)were researched from September 2016 to September 2020.In operation,exposing the tragal pointer,dissecting facial nerve trunk.In the coronal position,to measure the distance of extratemporal segment of the facial nerve trunk to the tip of the tragal pointer.And to explore the application of clinical operation.Results During the operation,it was found that the position of the facial nerve trunk in the lateral segment of the temporal bone was relatively constant,the cartilage tip of the ear canal outside the coronal position was a round center,and the coordinates of the superior nerve trunk in the coronal position(0.62 to 1.13 cm,0.59 to 1.11 cm).Conclusion In the coronal position,the positing of the tragal pointer and the extratemporal segment of facial nerve trunk are relative constancy.It has guiding significance for surgical operation.
