摘 要:目的探讨血清中25-羟基维生素D_(3)[25(OH)D_(3)]、N末端B型脑钠肽原(NT-proBNP)和白介素-6(IL-6)在川崎病患儿中的表达水平以及诊断价值。方法选取2018年1月至2020年5月于台州市中心医院儿科收治的66例川崎病患儿作为病例组,根据冠状动脉是否受损分为损伤组(n=30)和非损伤组(n=36),同时选取同期就诊的普通发热患儿66例作为对照组。检测所有对象血清中25(OH)D_(3)、NT-proBNP和IL-6水平,通过Pearson相关分析各指标间的关系,通过受试者工作曲线(ROC)分析诊断价值。结果病例组血清25(OH)D_(3)水平显著低于对照组,NT-proBNP以及IL-6水平显著高于对照组,差异均具有统计学意义(t值分别为21.734、38.173、41.382,均P<0.05);损伤组患儿25(OH)D_(3)水平显著低于非损伤组,NT-proBNP以及IL-6水平显著高于非损伤组,差异均具有统计学意义(t值分别为13.472、14.282、20.194,均P<0.05);患儿25(OH)D_(3)水平与NT-proBNP水平、IL-6水平均呈负相关(r值分别为-0.468、-0.429,均P<0.05),NT-proBNP水平与IL-6水平呈正相关(r=0.518,P<0.05);三者联合检测对川崎病诊断的ROC曲线下面积(AUC=0.939)显著大于三者单独诊断,同时三者联合检测灵敏度(84.13%)以及特异度(94.25%)显著提高,差异均具有统计学意义(均P<0.05)。结论 25(OH)D_(3)在川崎病患儿血清中表达降低,NT-proBNP和IL-6表达升高,三者联合检测可显著提高川崎病的诊断,具有重要的临床价值。Objective To investigate the expression levels of 25-hydroxyvitamin D_(3) [25(OH)D_(3)],N-terminal B-type pro-brain natriuretic peptide(NT-proBNP) and interleukin-6(IL-6) in children with Kawasaki disease and their diagnostic values.Methods A total of 66 children with Kawasaki disease admitted to the Department of Pediatrics of Taizhou Central Hospital from January 2018 to May 2020 were selected as the case group.According to whether the coronary artery was damaged, they were divided into injury group(n=30) and non-injury group(n=36).At the same time, 66 children with common fever who admitted to the hospital during the same period were selected as the control group.The serum levels of 25(OH)D_(3),NT-proBNP and IL-6 were detected in all subjects, the relationship between the indicators was analyzed by Pearson correlation, and the diagnostic value was analyzed by receiver operating characteristic(ROC) curve.Results The serum level of 25(OH)D_(3) in case group was significantly lower than that in control group, and the levels of NT-proBNP and IL-6 were significantly higher than those in control group, with statistical significances(t values were 21.734,38.173 and 41.382,respectively, all P<0.05).The level of 25(OH)D_(3) in the injury group was significantly lower than that in the non-injury group, and the levels of NT-proBNP and IL-6 were significantly higher than those in the non-injury group, with statistical significances(t values were 13.472,14.282 and 20.194,respectively, all P<0.05).The level of 25(OH)D_(3) was negatively correlated with the level of NT-proBNP and IL-6(r values were-0.468 and-0.429,respectively, both P<0.05),and the level of NT-proBNP was positively correlated with the level of IL-6(r=0.518,P<0.05).The area under the ROC curve(AUC=0.939) for the diagnosis of Kawasaki disease by the combined detection of the three indicators was significantly higher than that by three detection methods alone, and the sensitivity(84.13%) and specificity(94.25%) of the combined detection methods were significant
