摘 要:目的探究雾化吸入N-乙酰半胱氨酸治疗慢性阻塞性肺疾病(慢阻肺)急性加重期的临床疗效。方法以该院2018年6月—2020年5月收治的115例慢阻肺急性加重期患者为研究对象,以随机数表法分为两组,甲组患者行常规治疗,乙组在甲组基础上行雾化吸入N-乙酰半胱氨酸治疗,对比两组症状消失时间、治疗前后肺功能指标、血气功能指标变化情况。结果乙组呼吸困难、肺部啰音、咳嗽、喘息消失时间分别为(2.18±0.08)、(3.72±0.12)、(2.05±0.18)、(2.12±0.12)d,均短于甲组,差异有统计学意义(t=119.386、17.333、48.610、19.668,P<0.05);乙组治疗后FEV1、FVC、FEV1/FVC分别为(1.58±0.35)L、(3.15±0.38)L、(75.89±3.18)%,均高于甲组,差异有统计学意义(t=6.774、16.714、23.899,P<0.05);乙组治疗后PaCO_(2)为(42.15±0.28)mmHg,低于甲组,PaO_(2)为(88.18±4.18)mmHg,高于甲组,差异有统计学意义(t=112.074、35.683,P<0.05)。结论雾化吸入N-乙酰半胱氨酸治疗慢阻肺急性加重期的临床疗效显著,利于改善肺功能与血气状态。Objective To explore the clinical effect of nebulized inhalation of N-acetylcysteine in the treatment of acute exacerbation of chronic obstructive pulmonary disease.Methods 115 patients with acute exacerbation of chronic obstructive pulmonary disease admitted in the hospital from June 2018 to May 2020 were used as the research objects.They were divided into two groups by random number table.The patients in group A received conventional treatment,and the patients in group B based on the group A,added basic ascending aerosol inhalation of N-acetylcysteine treatment,comparing the time of symptom disappearance,lung function index and blood gas function index change between the two groups before and after treatment.Results The disappearance time of dyspnea,pulmonary rales,cough and wheezing in group B were(2.18±0.08)d,(3.72±0.12)d,(2.05±0.18)d,and(2.12±0.12)d,respectively,which were shorter than those of group A,the difference was statistically significant(t=119.386,17.333,48.610,19.668,P<0.05);after treatment in group B,FEV1,FVC,FEV1/FVC were(1.58±0.35)L,(3.15±0.38)L,(75.89±3.18)%,respectively,all were higher than group A,the difference was statistically significant(t=6.774,16.714,23.899,P<0.05);after treatment in group B,PaCO_(2) was(42.15±0.28)mmHg,which was lower than group A,and PaO_(2) was(88.18±4.18)mmHg,which was higher than group A,the difference was statistically significant(t=112.074,35.683,P<0.05).Conclusion Nebulized inhalation of N-acetylcysteine has a significant clinical effect in the treatment of acute exacerbation of chronic obstructive pulmonary disease,which is beneficial to improve lung function and blood gas status.
