从升阳通玄论治糖尿病周围神经病变

摘　　要：河间学派与易水学派在承袭《黄帝内经》理论体系的基础上,通过理论创新与方药创新,弥补了《黄帝内经》气化思想的不足,完善了其理法方药运用体系。通过深入研习,我们提出气化的最佳状态,即平人的3个标准:足、畅、衡;明确指出玄府气化理论体现了天人一体的整体观,既可用于指导外感,亦可用于指导内伤类疾病的治疗。以糖尿病周围神经病变(DPN)为例,笔者认为其基本病机是玄府气化失司,因此提出了针对性治疗大法即升阳通玄法。在升阳通玄法指导下,将易水学派两大名方升阳散火汤与元戎四物汤(桃红四物汤)结合,创立了达玄饮,并在升阳通玄理论指导下对方义进行了重新解读,充分展现了燕赵医学的特色与优势。On the basis of inheriting the theoretical system of Huang Di Nei Jing,Hejian School and Yishui School made up for qi transformation thought in Huang Di Nei Jing and perfected the application system of principle-method-recipe-medicinal through theoretical and prescription innovation.Through in-depth study,we put forward the best state of qi transformation,that is,the three standards of normal people:sufficiency,smoothness and balance,and clearly pointed out that qi transformation of sweat pore theory embodies the holistic view of the unity of heaven and man,which can be used to guide both exogenous contraction and internal injury diseases.Taking diabetic peripheral neuropathy(DPN)as an example,we think that its basic pathogenesis is the failure in qi transformation of sweat pore,so we put forward the targeted treatment method,namely Shengyang Tongxuan method(yang-ascending sweat-pore-unblocking method).Under the guidance of this method,Da Xuan Yin(Sweat-pore-Reaching Decoction)was established by combining Sheng Yang San Huo Tang(Yang-ascending Fire-purging Decoction)with Yuan Rong Si Wu Tang or Tao Hong Si Wu Tang(Four Ingredients Decoction),two famous prescriptions of Yishui School.Under the guidance of Shengyang Tongxuan theory,this paper reinterprets the meaning of recipe,which fully shows the characteristics and advantages of Yanzhao medicine.
