新生儿窒息血糖和血电解质变化的临床分析

摘  要:目的探讨新生儿窒息程度与血糖及血电解质紊乱的关系。方法115例窒息新生儿,根据Apgar评分分为轻度窒息组(80例)和重度窒息组(35例)。两组患儿入院后检测血糖和血电解质钾、钠、氯和钙,均给予相同的监护治疗。比较两组患儿血糖和血电解质情况。结果轻度窒息组患儿血糖水平及低血糖、高血糖、血糖紊乱发生率分别为(4.23±1.51)mmol/L、11.25%、8.75%、20.00%,均低于重度窒息组的(5.76±1.35)mmol/L、28.57%、37.14%、65.71%,差异具有统计学意义(P<0.05)。两组患儿血钾水平比较差异无统计学意义(P>0.05);轻度窒息组患儿血钠、血氯、血钙水平均高于重度窒息组,差异具有统计学意义(P<0.05)。结论新生儿窒息程度越重,血糖紊乱及血钠、血氯和血钙浓度降低越明显,临床应密切监测血糖及血电解质变化,及时对症治疗,防止病情恶化。Objective To discuss the correlation between the degree of neonatal asphyxia and blood glucose and blood electrolyte imbalance.Methods A total of 115 asphyxia newborns were divided into mild asphyxia group(80 cases)and severe asphyxia group(35 cases)according to Apgar score.After admission,both groups were detected blood glucose and blood electrolyte potassium,sodium,chlorine and calcium,and given the same monitoring treatment.The blood glucose and blood electrolytes of the two groups were compared.Results The blood glucose levels and the incidence of hypoglycemia,hyperglycemia,and blood glucose disturbances in the mild asphyxia group were(4.23±1.51)mmol/L,11.25%,8.75%,and 20.00%,respectively,which were lower than(5.76±1.35)mmol/L,28.57%,37.14%,65.71%in the severe asphyxia group,and the difference was statistically significant(P<0.05).There was no statistically significant difference in blood potassium level between the two groups(P>0.05).The levels of blood sodium,chlorine and calcium of mild asphyxia group were higher than those of severe asphyxia group,and the difference was statistically significant(P<0.05).Conclusion The more severe the asphyxia of the newborn,the more obvious the blood glucose disorder and the decrease of blood sodium,blood chloride and blood calcium concentration.The clinic should closely monitor the changes of blood glucose and blood electrolytes,and provide timely symptomatic treatment to prevent the deterioration of the disease.
