疮灵液对非哺乳期乳腺炎创面炎症的影响研究

摘　　要：目的探讨疮灵液外用治疗局部破溃非哺乳期乳腺炎患者疗效及对创面炎症的影响。方法选择南京中医药大学附属医院乳腺外科2018年1月—2019年6月收治的64例局部脓肿、窦道破溃期非哺乳期乳腺炎患者,根据采用的治疗方案不同分为2组,对照组33例给予常规治疗,疮灵液组31例给予疮灵液外用,比较2组治疗前和治疗4周后创面症状评分、创面组织炎症指标[肿瘤坏死因子-α(TNF-α)、干扰素-γ(IFN-γ)、白细胞介素-1β(IL-1β)、白细胞介素-2(IL-2)、白细胞介素-6(IL-6)]水平,Spearman分析炎症指标与创面症状的相关性,评估2组治疗4周后总体疗效并记录不良反应发生情况。结果治疗后,2组患者肿块范围、引流物性质、引流量、创周皮肤温度、创周肤色以及疼痛评分均明显降低(P均<0.05),且疮灵液组均明显低于对照组(P均<0.05);疮灵液组创面组织中TNF-α、IFN-γ、IL-1β、IL-2、IL-6水平及对照组IL-1β、IL-2、IL-6水平均明显下降(P均<0.05),且疮灵液组TNF-α、IFN-γ、IL-1β、IL-2水平均明显低于对照组(P均<0.05);创面组织中IL-1、IL-2、IL-6、IFN-γ、TNF-α水平均与创面症状呈正相关性(P均<0.05)。治疗4周后,疮灵液组总有效率为100%(31/31),对照组为60.6%(20/33),疮灵液组明显高于对照组(P<0.05);2组仅有医用胶带过敏发生,均更换辅料后症状消失。结论疮灵液外用干预局部破溃非哺乳期乳腺炎患者,可通过减轻创面组织炎症反应,有效改善局部症状,提高疗效。Objective It is to explore the curative effect of external application of Chuangling Liquid in the treatment of non-lactating mastitis patients with local ulceration and its influence on wound inflammation.Methods Sixty-four cases of non-lactating mastitis patients with local abscess and sinus rupture who were admitted to the Department of Breast Surgery,Affiliated Hospital of Nanjing University of Traditional Chinese Medicine from January 2018 to June 2019 were selected.They were divided into 2 groups according to the different treatment schemes used.33 cases in the control group were treated with conventional treatment,and 31 cases in Chuangling Liquid group were given external application of Chuangling Liquid.The wound symptom scores,the levels of wound tissue inflammation indexes[tumor necrosis factor-α(TNF-α),interferon-γ(IFN-γ),interleukin-1β(IL-1β),interleukin-2(IL-2),interleukin-6(IL-6)]were compared between the two groups before and 4 weeks after treatment.The correlation between inflammation indicators and wound symptoms was analyzed by Spearman,and the overall efficacy after 4 weeks of treatment was evaluated and the adverse reactions in the two groups were recorded.Results After treatment,the size of the tumor,the nature of the drainage,the volume of drainage,the skin temperature and the skin color around the wound,and the pain score of the two groups were significantly reduced(all P<0.05),and the above indicators in the Cangling Liquid group were significantly lower than those in the control group(all P<0.05);the levels of TNF-α,IFN-γ,IL-1β,IL-2,and IL-6 in the wound tissue of the Chuangling Liquid group and the levels of IL-1β,IL-2,and IL-6 in the control group were significantly decreased(P<0.05),and the levels of TNF-α,IFN-γ,IL-1βand IL-2 in the Cangling Liquid group were significantly lower than those in the control group(all P<0.05);the levels of IL-1 and IL-2.IL-6,IFN-γ,TNF-αin the wound tissue were positively correlated with wound symptoms(all P<0.05).After 4 weeks of treatment,
