中西药联合导引治疗病毒性心肌炎后房性早搏疗效观察

摘　　要：目的探讨中西药联合导引治疗病毒性心肌炎所致房性早搏的临床疗效。方法选择2018年2月—2020年2月陕西中医药大学附属医院治疗的病毒性心肌炎后房性早搏患者61例,采用随机数字表法将患者随机分为对照组31例、研究组30例。对照组给予常规西药(盐酸曲美他嗪片、辅酶Q10片)+桂枝甘草龙骨牡蛎汤治疗,研究组在上述基础上联合导引治疗。2组均以治疗6周为1个疗程。分别记录治疗过程中及治疗后相应时段房性早搏频次、心脏超声、睡眠质量指数评分、中医症状评分,比较2组临床疗效。结果与治疗前比较,2组治疗3,6周后的房性早搏频次均明显减少(P均<0.05),2组治疗6周后的房性早搏频次明显少于治疗3周后(P均<0.05),且研究组均明显少于同期对照组(P均<0.05)。与治疗前比较,2组治疗6周后左心室舒张末期的前/后径、长径均明显变短(P均<0.05);与对照组比较,研究组治疗后左心室舒张末期的前后径、长径均更短(P均<0.05);2组治疗前后左心室舒张末期左右径无显著变化(P均>0.05)。2组患者治疗6周后PSQI评分、中医症状评分均较治疗前显著降低(P均<0.05),且研究组治疗后均明显低于对照组(P均<0.05)。对照组与研究组治疗后的临床总有效率分别为86.4%(25/31)、96.7%(29/30),研究组明显高于对照组(P<0.05)。结论中西药联合导引治疗能够显著减少病毒性心肌炎所致房性早搏患者房性早搏频次,改善睡眠状况,减轻症状,间接抑制左心室心肌重构,提高临床疗效。Objective It is to explore the clinical efficacy of combined Chinese and western medicine with guided therapy in the treatment of atrial premature beats caused by viral myocarditis.Methods Sixty-one patients with atrial premature beats caused by viral myocarditis treated in the Affiliated Hospital of Shaanxi University of Traditional Chinese Medicine from February 2018-February 2020 were selected and randomly divided into a control group of 31 cases and a study group of 30 cases by using a random number table method.The control group was treated with conventional western medicine(trimetazidine hydrochloride tablets,coenzyme Q10 tablets)+Guizhi Gancao Longgu Muli Decoction,and the study group was treated with guided therapy on the basis of the treatment in the control group.Both groups were treated for a course of treatment of 6 weeks.The frequency of premature atrial beats,echocardiography,sleep quality index scores,and TCM symptom scores were recorded during the treatment process and the corresponding period after treatment,and the clinical effects of the two groups were compared.Results Compared with before treatment,the frequency of atrial premature beats after 3 and 6 weeks of treatment in the 2 groups was significantly reduced(P<0.05),and the frequency of atrial premature beats in the 2 groups after 6 weeks of treatment was significantly less than that after 3 weeks of treatment(P both<0.05),and the study group was significantly less than the control group during the same period(P<0.05).Compared with before treatment,the anterior/posterior diameter and long diameter of the left ventricle in the two groups were significantly shorter after 6 weeks of treatment(P<0.05);compared with the control group,the anterior/posterior diameter and long diameters of the left ventricle in the study group were shorter after treatment(both P<0.05);the left and right end-diastolic diameters of the left ventricle did not change significantly before and after treatment in the two groups(both P>0.05).After 6 weeks of treatment,the
