浅表淋巴结结核诊断中采用彩色多普勒超声的临床诊断意义分析

摘　　要：目的:探讨彩色多普勒超声在浅表淋巴结结核诊断中的应用价值。方法:2019年5月-2020年5月收治浅表淋巴结结核患者78例,随机分为两组,各39例。对照组采用CT检查;观察组采用彩色多普勒超声检查。比较两组检查结果与临床病理检查结果的相符程度。结果:观察组灵敏度和符合率显著高于对照组,差异有统计学意义(P<0.05);图像显示,患者颈部、颌下、腮腺处及腋下均显示低回声结节,大小不一;其中椭圆形结节边界、皮、髓较为清晰,髓质减少或消失,可见淋巴门;圆形结节边界相较不够清晰,且回声表现不均匀,同样可见淋巴门,但存在少数移位;团块状结节明显肿大,相较于内部,周边血流更丰富,且脓肿内部不存在血流信号。结论:相较于CT检查,彩色多普勒超声在浅表淋巴结结核诊断中更有价值,获取的病变数据详尽,灵敏度、符合率高。Objective:To explore the value of color Doppler ultrasound in the diagnosis of superficial lymph node tuberculosis.Methods:From May 2019 to May 2020,78 patients with superficial lymph node tuberculosis were selected,they were randomly divided into the two groups with 39 cases in each group.The control group received CT examination.The observation group received color Doppler ultrasound examination.We compared the degree of agreement between the two groups'examination results and the clinical pathological examination results.Results:The sensitivity and coincidence rate of the observation group were significantly higher than those of the control group,the difference was statistically significant(P<0.05).The images showed that the patient's neck,submandibular,parotid gland and axillary showed hypoechoic nodules of different sizes.Among them,the elliptic nodule boundary,skin,and medulla were clear,the medulla was reduced or disappeared,and the lymphatic hilum was visible.The round nodule boundary is relatively unclear,and the echo is not uniform.The lymphatic hilum is also visible,but there are a few displacement.The mass-shaped nodules were obviously enlarged,and the peripheral blood flow was more abundant than the inside,and there was no blood flow signal inside the abscess.Conclusion:Compared with CT examination,color Doppler ultrasound is more valuable in the diagnosis of superficial lymph node tuberculosis,with detailed lesion data,high sensitivity and high coincidence rate.
