基于CICARE沟通模式在胫骨平台骨折手术患者中的应用

摘　　要：目的探讨基于CICARE沟通模式护理在胫骨平台骨折(FTP)手术患者中的应用效果。方法前瞻性纳入海南省中医院2018年7月—2020年3月收治的107例FTP手术患者为研究对象,根据住院单双号,将双号患者53例纳入A组、单号患者54例纳入B组,A组接受常规护理,B组接受基于CICARE沟通模式护理。于术前、出院当天,比较两组负面情绪及护理满意度;术后随访6个月,比较两组术前、术后6个月膝关节功能。结果两组术前汉密尔顿焦虑(HAMA)、汉密尔顿抑郁(HAMD)量表评分比较,差异无统计学意义(P>0.05);两组出院当天HAMA和HAMD评分均低于术前,且B组HAMA、HAMD评分低于A组,差异有统计学意义(P<0.05);B组护理满意度高于A组,差异有统计学意义(P<0.05);两组术后6个月膝关节功能评分均高于术前,差异有统计学意义(P<0.05),两组膝关节功能比较,差异无统计学意义(P>0.05)。结论FTP手术患者采用基于CICARE沟通模式护理,利于减轻患者负面情绪,提升护理满意度。Objective To explore the application effect of based on CICARE communication mode nursing in patients with fracture of tibial plateau(FTP)surgery.Methods A prospective study of 107 FTP patients admitted to Hainan Hospital of Traditional Chinese Medicine from July 2018 to March 2020 was included,according to the odd-even number of hospitalized patients,53 patients with even number were included in group A and 54 patients with single number were included in group B;group A were received routine care and group B were received based on CICARE communication mode care.Before surgery and on the day of discharge,negative emotions,nursing satisfaction was compared between the two groups;the knee function of the two groups was compared before and after six months of follow-up.Results There was no significant difference in the scores of hamilton anxiety(HAMA)and hamilton depression(HAMD)scale between the two groups(P>0.05);the scores of HAMA and HAMD in the two groups on the day of discharge were lower than those before operation,and the scores of HAMA and HAMD in group B were lower than those in group A,with statistical significance(P<0.05);the nursing satisfaction of group B was higher than group A,and the difference was statistically significant(P<0.05);the knee function scores of both groups six months after surgery were higher than those before surgery,and the differences were statistically significant(P<0.05).There was no statistically significant difference in knee function between the two groups(P>0.05).Conclusion FTP patients adopt CICARE communication mode nursing,which is beneficial to reduce patients’negative emotions and improve nursing satisfaction.
