摘 要:目的:研究补肾活血壮骨汤对膝骨性关节炎(knee osteoarthritis,KOA)模型大鼠的作用。方法:将150只雄性SD大鼠随机分成正常组、模型组及补肾活血壮骨汤低剂量(0.010 g·kg^(-1))、中剂量(0.015 g·kg^(-1))和高剂量(0.020 g·kg^(-1))组,每组30只。除正常组外,其余大鼠均以关节腔注射木瓜蛋白酶复制KOA模型。造模成功后正常组与模型组予以生理盐水灌胃,其余大鼠均予以补肾活血壮骨汤灌胃,干预4周,检测大鼠压痛阈值、足肿胀度、肿胀消退时间,全血高切、血浆黏度,血清中各项基质金属蛋白酶(matrix metalloproteinase,MMP)、炎症因子水平,观察关节软骨病理变化。结果:补肾活血壮骨汤组大鼠压痛阈值明显升高、足肿胀度明显减轻、肿胀消退时间明显缩短;全血高切、血浆黏度明显降低;血清中MMP-1、MMP-3、MMP-13、白细胞介素-6(interleukin-6,IL-6)、肿瘤坏死因子-α(tumor necrosis factor-α,TNF-α)、白细胞介素-1β(interleukin-1β,IL-1β)水平明显降低,基质金属蛋白酶抑制剂-1(matrix metalloproteinase inhibitor-1,TIMP-1)水平明显升高,且呈剂量依赖性。结论:补肾活血壮骨汤治疗KOA模型大鼠可能与改善血液流变学、抑制炎性因子有关。Objective:To study the effect of Bushen Huoxue Zhuanggu Decoction on knee osteoarthritis(KOA)model rats.Methods:150 SD rats were divided into normal group,model group,Bushen Huoxue Zhuanggu Decoction low-dose group(0.010 g·kg^(-1)),medium-dose group(0.015 g·kg^(-1))and high-dose group(0.020 g·kg^(-1))according to random number table,with 30 rats in each group.KOA rat model was made by papain injection in the knee cavity of rats except the normal group.After successful modeling,normal group and model group were given normal saline intragastric gavage.The other rats were treated with Bushen Huoxue Zhuanggu Decoction by intragastric administration.All rats were treated for 4 weeks,and the differences in hemorheology,matrix metalloproteinase(MMP),expression of inflammatory factors,tenderness threshold,the degree of foot swelling,time of swelling subsidence,changes of articular cartilage and other aspects were analyzed.Results:In Bushen Huoxue Zhuanggu Decoction group,tenderness threshold was significantly increased,foot swelling was significantly reduced,and swelling subsided time was significantly shortened;whole blood high shear viscosity and plasma viscosity was significantly reduced;serum MMP-1,MMP-3,MMP-13,and interleukin-6(IL-6),tumor necrosis factor-α(TNF-α),interleukin-1β(IL-1β)levels were significantly reduced,matrix metalloproteinase inhibitor-1(TIMP-1)was significantly increased in a dose-dependent manner.Conclusion:The mechanism of Bushen Huoxue Zhuanggu Decoction in treating KOA model rats may relate to blood rheology improvement and inflammatory factors inhibition.
