白头翁汤联合透灸对湿热型溃疡性结肠炎患者炎症细胞因子和免疫功能的影响

摘　　要：目的:观察白头翁汤联合透灸法治疗湿热型溃疡性结肠炎的临床疗效。方法:选择90例溃疡性结肠炎(湿热证)患者随机分为对照组(给予美沙拉嗪肠溶片)和观察组(给予白头翁汤联合透灸法)。治疗后,比较两组患者临床疗效、中医证候积分、Mayo评分、炎症因子水平及T淋巴细胞亚群。结果:治疗后,对照组有效率为71.11%,观察组有效率为93.33%,两组比较,差异有统计学意义(P<0.05)。治疗后,两组患者中医证候积分、Mayo评分均显著下降,且观察组低于对照组,差异均有统计学意义(P<0.05)。治疗后,两组患者白细胞介素-6、白细胞介素-8、肿瘤坏死因子-α水平均显著下降,且观察组以上指标显著低于对照组,差异均有统计学意义(P<0.05)。治疗后,两组患者CD3^(+)、CD8^(+)水平均显著下降,CD4^(+)、CD4^(+)/CD8^(+)水平显著升高;观察组CD3^(+)、CD8^(+)水平均显著低于对照组,CD4^(+)、CD4^(+)/CD8^(+)水平显著高于对照组,差异均有统计学意义(P<0.05)。结论:白头翁汤联合透灸法治疗湿热型溃疡性结肠炎疗效显著,可有效缓解患者各项症状,减轻肠道炎症反应,提高机体免疫功能。Objective:To observe the clinical effect of Baitouweng decoction combined with penetration moxibustion on Damp-heat type ulcerative colitis(UC).Methods:90 UC patients(Damp-heat type)were randomly divided into control group(treated with mesalazine enteric-coated tablets)and observation group(treated with Baitouweng decoction combined with penetration moxibustion).The clinical efficacy,TCM symptom score,Mayo score,inflammatory factor levels,and T lymphocyte subsets score were evaluated and compared between the two groups after treatment.Results:After treatment,the effective rate was 71.11%in the control group and 93.33%in the treatment group,the difference between the two groups was statistically significant(P<0.05).The TCM symptom score and Mayo score in both groups decreased remarkably,and the observation group was lower than the control group,the difference was statistically significant(P<0.05)The levels of IL-6,IL-8,and TNF-αin the two groups decreased significantly,and the observation group was significantly lower than those in the control group,the differences were statistically significant(P<0.05).The levels of CD3^(+)and CD8^(+)in the two groups decreased significantly,and the levels of CD4^(+),CD4^(+)/CD8^(+)increased significantly;the levels of CD3^(+)and CD8^(+)in the observation group were significantly lower than that in the control group,and the levels of CD4^(+)and CD4^(+)/CD8^(+)were significantly higher than that in the control group.The differences were statistically significant(P<0.05).Conclusion:Baitouweng decoction combined with penetration moxibustion can effectively relieve the symptoms of patients with damp-heat type UC,reduce the intestinal inflammatory reaction,and improve body immunity.
