不同栓塞面积对改善脾亢外周血细胞减少的相关性研究

摘　　要：目的:探析不同栓塞面积对改善脾亢外周血细胞减少的相关性。方法:2018-2020年收治肝功能child-pugh分级为B级的脾亢患者90例,按不同栓塞范围分为三组,各30例。Ⅰ组栓塞面积为40%~49%,Ⅱ组为50%~59%,Ⅲ组为60%~69%;术前进行血常规白细胞、血小板的检查,术后复查24 h、2周、1个月、2个月的白细胞、血小板变化,分析相关性。结果:Ⅰ组患者术后白细胞、血小板升高倍率最高,其次为Ⅲ组,最后是Ⅱ组。三组轻度疼痛比较,Ⅰ组>Ⅱ组>Ⅲ组,差异有统计学意义(P<0.05);中度、重度疼痛比较,Ⅰ组<Ⅱ组<Ⅲ组,差异有统计学意义(P<0.05)。结论:部分脾栓塞可以改善脾功能,使外周血细胞增高,栓塞范围在40%~49%的患者可以满足临床改善脾功能的需要。Objective:To explore the correlation between different embolism areas and peripheral blood cell reduction in patients with hypersplenism.Methods:From 2018 to 2020,90 cases of patients with hypersplenism with liver function child-pugh of grade B were enrolled,they were divided into three groups according to different embolism ranges,with 30 cases in each group.The embolization area of groupⅠwas 40%~49%,groupⅡwas 50%~59%,groupⅢwas 60%~69%.Routine blood examination of white blood cells and platelets were performed before surgery,the changes of leukocyte and platelet after 24 h,2 weeks,1 month and 2 months of surgery were examined again in order to analyze their correlation.Results:The highest rates of leukocytes and platelets were found in groupⅠ,followed by groupⅡ,the last was groupⅢ.The mild pain of the three groups was compared,groupⅠ>groupⅡ>groupⅢ,the difference was statistically significant(P<0.05).The moderate and severe pain of the three groups was compared,groupⅠ<groupⅡ<groupⅢ,the difference was statistically significant(P<0.05).Conclusion:Partial splenic embolism can improve splenic function and increase peripheral blood cells.Patients with embolization at the range of 40%~49%can meet the needs of clinical improvement of splenic function.
