门冬胰岛素联合地特胰岛素治疗妊娠糖尿病患者的临床效果分析

摘　　要：目的分析门冬胰岛素联合地特胰岛素治疗妊娠糖尿病的临床疗效。方法选取62例妊娠糖尿病患者,采用随机数字表法分成研究组与对照组,每组31例。对照组应用门冬胰岛素进行治疗,研究组应用门冬胰岛素联合地特胰岛素治疗。比较两组治疗前后血糖指标、血糖达标时间、分娩方式及母婴不良结局发生情况。结果治疗前,两组患者空腹血糖、餐后2 h血糖、糖化血红蛋白(HbA_(l)c)水平比较,差异无统计学意义(P>0.05);治疗后,两组患者空腹血糖、餐后2 h血糖、HbA_(l)c水平均明显低于治疗前,且研究组患者低于对照组,差异具有统计学意义(P<0.05)。研究组空腹血糖达标时间、餐后2 h血糖达标时间及二者血糖全部达标时间均明显短于对照组,差异有统计学意义(P<0.05)。研究组剖宫产率为22.58%,明显低于对照组的54.84%,差异有统计学意义(P<0.05)。研究组母婴不良结局发生率为19.35%,明显低于对照组的48.39%,差异有统计学意义(P<0.05)。结论门冬胰岛素联合地特胰岛素治疗妊娠糖尿病,可改善患者血糖控制效果,缩短血糖达标时间,改善患者妊娠结局,值得临床推广与应用。Objective To analyze the clinical efficacy of insulin aspartate combined with insulin detemine in the treatment of gestational diabetes mellitus.Methods 62 patients with gestational diabetes mellitus were divided into study group and control group by random number table method,with 31 cases in each group.The control group was treated with insulin aspartate,and the study group was treated with insulin aspartate combined with insulin despecil.The blood glucose indexes,the time of blood glucose normalization,delivery mode and the incidence of adverse outcomes of mother and child were compared between the two groups before and after treatment.Results Before treatment,there were no significant differences in the levels of fasting blood glucose,2 h postprandial blood glucose and glycated hemoglobin(HbA_(l)c)between 2 groups(P>0.05).After treatment,the levels of fasting blood glucose,2 h postprandial blood glucose and HbA_(l)c in 2 groups were significantly lower than before treatment,and the study group was lower than the control group,the difference was statistically significant(P<0.05).The standard time of fasting blood glucose,2 h postprandial blood glucose and all the standard time of both groups in the study group were significantly shorter than those in the control group,and the difference was statistically significant(P<0.05).The cesarean section rate of the study group was 22.58%,which was significantly lower than that of the control group(54.84%),and the difference was statistically significant(P<0.05).The incidence of adverse outcomes was 19.35%in the study group,which was significantly lower than 48.39%in the control group,and the difference was statistically significant(P<0.05).Conclusion Insulin aspartate combined with insulin detemer in the treatment of gestational diabetes mellitus can improve the effect of blood glucose control,shorten the time of blood glucose standard,and improve the pregnancy outcome of patients,which is worthy of clinical promotion and application.
