针刺治疗干眼症疗效观察及对患者生活质量、焦虑抑郁情绪的影响

摘　　要：目的观察针刺治疗干眼症的临床疗效,并探讨其对患者生活质量、焦虑抑郁情绪的影响。方法将72例干眼症患者随机分为针刺组和对照组,每组36例。针刺组取攒竹、太阳、丝竹空等穴位针刺治疗,对照组采用聚乙二醇滴眼液治疗。观察两组患者治疗前后眼表疾病指数、泪膜破裂时间、基础泪液分泌试验以及生活质量量表、医院焦虑抑郁量表的变化。结果针刺可降低患者眼表疾病指数,针刺治疗前后比较及与对照组组间比较,差异均有统计学意义(P<0.01);针刺可促进泪液分泌,延长泪膜破裂时间,治疗前后比较及与对照组组间比较差异均有统计学意义(P<0.01);针刺可提高患者生活健康状况,在生理职能、机体疼痛、总体健康、心理健康方面,针刺治疗前后比较及与对照组组间比较,差异均有统计学意义(P<0.05);针刺治疗能够改善患者焦虑情绪,针刺治疗前后比较及组间比较,差异均有统计学意义(P<0.01);针刺治疗对患者抑郁情绪无显著作用(P>0.05)。结论针刺治疗能有效改善干眼症患者的症状,延长泪膜破裂时间,提高泪量,同时还能改善患者的生活健康状况及焦虑情绪。Objective To observe the clinical efficacy of acupuncture in treating dry eye syndromes and discuss its effects on patients’quality of life(QOL)as well as anxious and depressed moods.Method Seventy-two patients with dry eye syndromes were randomized into an acupuncture group and a control group,with 36 cases in each group.Acupoints including Cuanzhu(BL2),Taiyang(EX-HN5)and Sizhukong(TE23)were selected for acupuncture treatment in the acupuncture group and the control group was intervened by Polyethylene glycol eye drops.Ocular surface disease index(OSDI),tear film break-up time(BUT),SchirmerⅠtest(SⅠT),36-item short-form health survey(SF-36),hospital anxiety and depression scale(HADS)were observed before and after treatment in the two groups.Result Acupuncture can reduce OSDI:the difference before and after acupuncture treatment and between the acupuncture and control groups after treatment were statistically significant(P<0.01);acupuncture can boost tear secretion and extend BUT:the differences before and after acupuncture treatment and between the two groups after treatment were statistically significant(P<0.01);acupuncture can improve the patients’life and health:comparing the domains of physical functioning,bodily pain,general health and mental health,the differences before and after acupuncture and the differences between the two groups after treatment were statistically significant(P<0.05);acupuncture can improve the anxious mood:the differences before and after acupuncture and between the acupuncture and control groups after treatment were statistically significant(P<0.01);the effect of acupuncture treatment on depressed mood was insignificant(P>0.05).Conclusion Acupuncture can effectively improve the symptoms in dry eye syndromes patients,such as extending BUT and increasing tear production,and it can also improve their life and health status and anxious mood.
