摘 要:目的:研究脂肪乳氨基酸(17)葡萄糖(11%)注射液对消化系统肿瘤切除术后血糖水平及并发症的影响。方法:选择2017年1月~2019年12月本院肿瘤科收治的90例行消化道系统肿瘤切除术患者作为研究对象,按照营养输注内容物的不同将其分为研究组和对照组,每组45例。研究组患者术后输注脂肪乳氨基酸(17)葡萄糖(11%)注射液,对照组患者术后输注常规配制的高氮、高热量营养袋。对比两组术后一般情况(首次排气时间、首次排便时间、KPS评分、住院时间、住院费用)、术后2、7天营养水平[视黄醇结合蛋白(RBP)、转铁蛋白(TRF)、前白蛋白(PA)]、血糖变化[空腹血糖(FPG)]、肝功能变化[谷草转氨酶(AST)、谷丙转氨酶(ALT)、碱性磷酸酶(ALP)],以及术后并发症发生情况。结果:研究组术后首次排气时间、首次排便时间均早于对照组,住院时间短于对照组(P<0.05)。研究组术后7天FPG水平低于术后2天,且低于对照组术后7天水平(P<0.05)。两组术后7天AST、ALT、ALP水平高于术后2天(P<0.05);两组术后7天AST、ALT、ALP水平组间比较无统计学差异(P>0.05)。研究组术后并发症总发生率(20.00%)低于对照组(40.00%,P<0.05)。结论:脂肪乳氨基酸(17)葡萄糖(11%)注射液能有效控制消化系统肿瘤切除术后血糖水平,降低术后并发症发生率,有利于患者术后恢复,值得临床推广应用。Objective:To study the effect of fat emulsion,amino acids(17)and glucose(11%)injection on blood glucose and complications after tumor resection of digestive system.Methods:From January 2017 to December 2019,a total of 90 patients who underwent tumor resection in the digestive tract in the department of oncology of our hospital were selected as the research objects.According to the different contents of nutrition infusion,they were divided into study group and control group,45 cases in each group.Study group was infused with fat emulsion,amino acids(17)and glucose(11%)injection after operation,while control group was infused with conventional preparation high nitrogen and high calorie nutrition bag after operation.Postoperative general conditions(first exhaust time,first defecation time,KPS score,duration of hospitalization and hospitalization costs),the levels of nutritional(RBP,TRF,PA),the change of blood glucose(FPG),the change of hepatic function(AST,ALT,ALP)on day 2 and day 7 after operation,and the incidence of postoperative complications in the two groups were compared.Results:The first exhaust time and first defecation time in study group were earlier than in control group,and the duration of hospitalization was shorter than that of the control group(P<0.05).The level of FPG in study group on day 7 was lower than that on day 2,and was lower than that in the control group on day 7(P<0.05).The levels of AST,ALT and ALP in the two groups on day 7 were higher than those in the two groups on day 2(P<0.05).There was no statistical difference between the levels of AST,ALT and ALP in the two groups on day 7(P>0.05).The total incidence of postoperative complications in study group(20.00%)was lower than in control group(40.00%,P<0.05).Conclusion:Fat emulsion,amino acids(17)and glucose(11%)injection can effectively control the blood glucose levels after tumor resection of the digestive system,and reduce the incidence of postoperative complications.It is beneficial to postoperative recovery of patients and worthy of c
