摘 要:目的探讨慢性肾脏疾病患者脑代谢物水平的变化。方法选取慢性肾脏疾病4期患者43例,按是否行血液透析分为非透析组23例和透析组20例,另选取同期健康体检者24例为健康对照组。对入选者行头颅磁共振平扫、磁共振波谱成像,通过相位矫正、拟合曲线得到脑代谢波谱。利用LC Model对波谱进行后处理,得出代谢物绝对浓度,观察脑代谢物水平变化。结果透析组脑代谢物相对浓度(mI/TCr)_(fwm)、(mI/TCr)_(bg)、(NAA/TCr)_(bg)低于对照组,差异有统计学意义(P<0.05)。透析组脑代谢物绝对浓度mI_(fwm)、Cho_(fwm)、TCr_(fwm)、TCr_(bg)、(NAA+NAAG)_(fwm)低于对照组,Cho_(fwm)低于非透析组,差异有统计学意义(P<0.05);非透析组TCr_(fwm)低于对照组,差异具有统计学意义(P<0.05)。非透析组中5例存在认知损害,透析组中6例存在认知损害。结论慢性肾脏疾病患者脑代谢物水平发生变化,认知功能存在一定程度损伤。Objective To investigate the changes of brain metabolites in patients with chronic kidney disease.Methods A total of 43 patients with chronic kidney disease stage 4 were selected and divided into non dialysis group(n=23)and dialysis group(n=20)according to whether they underwent hemodialysis or not.Another 24 healthy people in the same period of health screening were selected as healthy control group.Head MRI scan and magnetic resonance imaging were performed on the selected candidates,and the brain metabolism spectrum was obtained through phase correction and curve fitting.LC Model was used to post-process the spectrum to obtain the absolute concentration of metabolites.The changes of brain metabolite levels were observed.Results Relative brain metabolites concentrations of(mI/TCr)fwm,(mI/TCr)bg,and(NAA/TCr)bg in the dialysis group were lower than those in the control group with significant difference(P<0.05);absolute brain metabolites concentrations of mIfwm,Chofwm,TCrfwm,TCrbg and(NAA+NAAG)fwm in the dialysis group were lower than those in the control group;Chofwm in the dialysis group was significantly lower than that in the non-dialysis group(P<0.05);TCrfwm in the non-dialysis group was significantly lower than that in the control group(P<0.05).There were 5 cases with cognitive damage in the non-dialysis group,and 6 cases with cognitive damage in the dialysis group.Conclusion The level of brain metabolites in patients with chronic kidney disease has changed,and the cognitive function has a certain degree of damage.
