摘　　要：目的探讨经眶上外侧入路与经翼点入路动脉瘤夹闭术治疗颅内前循环动脉瘤患者的临床疗效。方法将68例颅内前循环动脉瘤患者按照手术入路不同分为眶上外侧组与翼点组各34例,眶上外侧组予以经眶上外侧入路动脉瘤夹闭术治疗,翼点组予以经翼点入路动脉瘤夹闭术治疗。比较两组手术相关指标,包括术后住院时间、切口长度、骨窗范围、术中失血量、手术时间。采用格拉斯哥预后量表评分等级评定两组预后情况,比较两组并发症发生率。结果眶上外侧组手术时间、术后住院时间显著短于翼点组(P<0.01),术中失血量显著少于翼点组(P<0.01),切口长度、骨窗范围显著小于翼点组(P<0.01)。眶上外侧组格拉斯哥预后量表评分分级优于翼点组(P<0.05),并发症发生率显著低于翼点组(P<0.05)。结论经眶上外侧入路动脉瘤夹闭术治疗颅内前循环动脉瘤效果显著,可加快康复进程,改善预后,降低并发症发生率。Objective To investigate the effect of clipping aneurysms through lateral supraorbital approach and pterion approach in the treatment of intracranial anterior circulation aneurysm.Methods A total of 68 patients with intracranial anterior circulation aneurysm were divided into lateral supraorbital group and pterion group according to different surgical approaches,with 34 cases each.The lateral supraorbital group received clipping aneurysms through lateral supraorbital approach,and the pterion group received clipping aneurysms through pterion approach.The operation-related indicators of the two groups were compared,including postoperative hospital stay,incision length,bone window range,intraoperative blood loss,and operation time.The prognosis of the two groups was assessed by the Glasgow Outcome Scale(GOS),and the incidence of complications between the two groups was compared.Results The operation time and postoperative hospital stay in the lateral supraorbital group were significantly shorter than those in the pterion group(P<0.01),the intraoperative blood loss was significantly less than that of the pterion group(P<0.01),and the length of the incision and the range of the bone window were significantly smaller than those of the pterion group(P<0.01).The GOS score of the lateral supraorbital group was better than that of the pterion group(P<0.05),and the complication rate was significantly lower than that of the pterion group(P<0.05).Conclusions The treatment of intracranial anterior circulation aneurysm by clipping aneurysms through lateral supraorbital approach is effective,which can accelerate the recovery process,improve prognosis and reduce the incidence of complications.
