摘 要:目的探讨MRI在新兵新训期间疲劳性骨折诊断中的应用价值。方法回顾性分析2017年9月至2020年10月解放军联勤保障部队第九四三医院新兵新训期间经临床诊断及随访证实的76例疲劳性骨折患者的临床资料,均行X线及MRI检查,分析两种检查的影像学特征。结果X线检查结果显示,76例患者中,19例有明显骨折征象,57例未见明确骨折改变。MRI检查结果显示,76例患者中,69例显示出疲劳性骨折的明显骨折线,7例骨折线显示较为模糊;76例均有骨髓水肿,且骨折线在T1WI与T2WI序列中均呈现低信号,STIR序列诊断疲劳性骨折的敏感性更高。MRI诊断疲劳性骨折的准确率达100%,而X线检查的准确率仅为25%。结论MRI在疲劳性骨折的早期诊断中有较高的诊断价值,可有效减少漏诊率及误诊率。Objective The application value of MRI in diagnosing fatigue fractures for new recruits during early training period was investigated.Methods The clinical data of 76 patients with fatigue fractures confirmed by clinical follow-up or pathology from September 2017 to October 2020 in the 943 Hospital of Liberation Army Strategic Support Force during new recruits'early raining period were retrospectively analyzed and all of them received X-ray and MRI examination.The imaging characteristics of the two examination modes were analyzed.Results The X-ray results showed that among 76 cases,19 cases had definite fracture symptoms,while 57 cases had no definite fracture change.The MRI results showed that among 76 cases,69 cases had definite fracture line,while 7 cases had blurred fracture line;all of the 76 cases had bone marrow edema and all the fracture lines appeared low signal intensity on T1WI and T2WI sequences,STIR were the most sensitive sequence in diagnosing fatigue fractures.The accuracy of MRI in diagnosing fatigue fractures proved to be 100%,while the value was 25%for X-ray.Conclusion MRI had higher diagnostic value in the early diagnosis of fatigue fractures,which could greatly reduce the rates of missed diagnosis and misdiagnosis.
