摘 要:目的研究320排CT低剂量扫描在小儿颅脑检查中的应用价值。方法选取2019年2月至2020年2月深圳市龙岗中心医院收治的120例临床高度怀疑的颅脑病变患儿,均拟行320排CT全脑扫描。采用随机数表法将患者分为A组(60例,常规剂量,250 mAs)和B组(60例,低剂量,90 mAs)。比较两组患儿的辐射剂量、噪声及图像质量评分情况,以及病灶诊断、病灶形态显示情况和定性诊断结果。结果A组患儿容积CT剂量加权指数(CTDIv)、全头颅扫描剂量长度乘积(DLP)、有效剂量(E)分别为(46.20±2.56)mGy、(650.00±28.54)mGy·cm、(1.36±0.20)mSv,明显高于B组的(16.87±1.20)mGy、(230.00±14.20)mGy·cm、(0.50±0.10)mSv,差异均有统计学意义(P<0.05);A组患儿的CT值为3.65±0.42,明显低于B组的5.72±0.71,差异有统计学意义(P<0.05);A组和B组患儿的图像质量优良率分别为96.67%、93.33%,差异无统计学意义(P>0.05);A组患儿发现颅内病灶44个,B组发现颅内病灶43个;两组患儿单个高密度灶、弥散性高密度灶、局限性低密度灶和脑沟脑回改变显示情况比较差异均无统计学意义(P>0.05);两组患儿病灶形态显示情况及颅内出血、外伤性腔梗、缺血缺氧性脑病、肿瘤定性诊断结果比较差异亦均无统计学意义(P>0.05)。结论小儿颅脑检查中行320排CT低剂量扫描,基本能满足临床诊断要求,且能减少小儿颅脑检查辐射。Objective To study the application value of low-dose scan of 320-slice CT in children's brain examination.Methods From February 2019 to February 2020,120 children with clinically highly suspected brain lesions admitted to Shenzhen Longgang Central Hospital were selected,all of which planned to receive 320-slice CT whole brain scan.The patients were randomly divided into group A(60 cases,conventional dose,250 mAs)and group B(60 cases,low dose,90 mAs).The radiation dose,noise,and image quality scores,as well as the diagnosis,morphological display and qualitative diagnosis results of the lesions were compared between the two groups.Results Volume CT dose-weighted index(CTDIV),whole head scan dose-length product(DLP),and effective dose(E)in group A were(46.20±2.56)mGy,(650.00±28.54)mGy·cm,(1.36±0.20)mSv,respectively,significantly higher than(16.87±1.20)mGy,(230.00±14.20)mGy·cm,(0.50±0.10)mSv in group B(P<0.05).The CT value of group A was 3.65±0.42,significantly lower than 5.72±0.71 in group B(P<0.05).The excellent and good rate of image quality in group A and group B were 96.67%and 93.33%respectively,and the difference was not statistically significant(P>0.05).There were 44 intracranial lesions in group A and 43 intracranial lesions in group B.There were no significant differences in single high-density foci,diffuse high-density foci,localized low-density foci,and sulcus and gyrus changes between the two groups(P>0.05).There was no significant difference between the two groups in the shape of lesions,intracranial hemorrhage,traumatic luminal infarction,hypoxic-ischemic encephalopathy,and qualitative diagnosis of tumor(P>0.05).Conclusion Low-dose scan of 320-slice CT can basically meet the requirements of clinical diagnosis,and can reduce the radiation of brain examination in children.
