摘 要:[目的 ]通过构建基于DRG的护理单元绩效管理评价体系,分析模型的可行性,为护理绩效精细化管理提供科学依据。[方法 ]从某院信息管理系统中导出2020年56个护理单元92, 013名出院患者病案首页信息与护理单元相关信息,首先建立以护理级别为主的绩效管理评价体系(模型一),其次引入国家卫健委版分组方案(CN-DRG),在此基础上构建护理级别与DRG相结合的评价体系(模型二),分析不同模型对护理单元绩效管理的影响。[结果 ]模型一56个护理单元中,3个(5.36%)位于优档(RSR> 0.5)、49个(87.50%)位于良档(RSR:0.3~0.5)、4个(7.14%)位于差档(RSR <0.3),各档差异有统计学意义(F=78.921,P <0.05);模型二中,19个(33.93%)护理单元位于优档(RSR> 0.5)、31个(55.35%)位于良档(RSR:0.3~0.5)、6个(10.70%)位于差档(RSR <0.3),各档差异具有统计学意义(F=43.128,P <0.05);各护理单元RSR平均变动6.4个顺位。[结论 ]引入DRG后护理绩效更能体现护理工作实际,不同护理单元绩效结构趋于公平,DRG以崭新的视角为护理绩效管理提供了新方法,对护理管理具有一定促进作用。Objective Through the construction of nursing unit performance management evaluation system based on DRG,the feasibility of the model was analyzed so as to provide scientific basis for fine management of nursing performance.Methods The first page of disease record information of 92013 discharged patients in 56 nursing units in 2020 was derived from the information management system of a hospital.Firstly,the performance management evaluation system based on nursing level(model 1)was established.Secondly,the National Health Commission version grouping scheme(cn-drg)was introduced.On this basis,the evaluation system combining nursing level and DRG(model 2)was constructed,analyzed the influence of different models on nursing unit performance management.Results 56 nursing units in model 1,3(5.36%)were in excellent grade(RSR>0.5),49(87.50%)were in good grade(RSR:0.3~0.5),4(7.14%)were in poor grade(RSR:<0.3),and there was statistically significant in differences(F=7&921fv0.05).In model 2,19(33.93%)nursing units were in excellent grade(RSR>0.5),31(55.35%)were in good grade(RSR:0.3~0.5)and 6(10.70%)nursing units were in the poor grade(RSR:<0.3).There was statistically significant in difference(F=43.128,P<0.05).The average change of RSR of each nursing unit was 6.4 cis-position.Conclusions Nursing unit performance successfully reflect actual nursing work after introducing DRG.The performance structure of different nursing units tends to fairness.DRG provides new way for unit performance management in a fresh perspective and has a certain role in promoting nursing management.
