摘 要:目的探究血清趋化素、过氧化物酶体增殖物激活受体α(peroxisome proliferator activated receptor alpha,PPAR-α)在老年慢性心力衰竭(chronic heart failure,CHF)患者血清中的表达水平及二者与预后的关系。方法选取广州医科大学附属第二医院2017年7月至2018年7月诊治的95例CHF患者纳入患病组,选取同期于该院体检的60例健康者纳入健康组。采用酶联免疫吸附测定检测两组研究对象血清趋化素、PPAR-α表达水平,并检测左心房内径(left atrial diameter,LAD)、左心室射血分数(left ventricular ejection fraction,LVEF)、左心室舒张末期内径(left ventricular end diastolic dimension,LVEDD)、左心室质量指数(left ventricular mass index,LVMI)等心功能指标;采用Pearson相关性分析探讨血清趋化素、PPAR-α表达水平与心功能指标的相关性;采用Kaplan-Meier生存曲线分析血清趋化素、PPAR-α表达水平与患者预后的关系。结果与健康组相比,患病组患者血清趋化素表达水平显著上调(P<0.05),PPAR-α表达水平显著下调(P<0.05);随着纽约心脏病协会(New York Heart Association,NYHA)心功能分级的增加,血清趋化素表达水平显著增加(均P<0.05),PPAR-α表达水平显著下降(均P<0.05);患病组患者LAD、LVEDD、LVMI均显著高于健康组(均P<0.05),LVEF显著低于健康组(P<0.05);Pearson相关性分析显示,CHF患者血清趋化素表达水平与PPAR-α表达水平呈负相关(r=-0.455,P<0.05);CHF患者血清趋化素表达水平与LAD、LVEDD、LVMI均呈正相关(均P<0.05),与LVEF呈负相关(P<0.05);血清PPAR-α表达水平与LAD、LVEDD、LVMI均呈负相关(均P<0.05),与LVEF呈正相关(P<0.05);Kaplan-Meier生存曲线分析显示,趋化素高表达组患者终点事件发生率显著高于趋化素低表达组(P<0.05),PPAR-α高表达组患者终点事件发生率显著低于PPAR-α低表达组(P<0.05)。结论老年CHF患者血清趋化素、PPAR-α表达水平与老年CHF患者预后有关,可能是CHF临床预�Objective To investigate the expression levels of serum chemerin and peroxisome proliferator activated receptor alpha(PPAR-α)in elderly patients with chronic heart failure(CHF)and their relationship with prognosis.Method From July 2017 to July 2018,95 patients with CHF in the Second Affiliated Hospital of Guangzhou Medical University were selected as disease group,60 healthy people with normal physical examination in the same period were selected as healthy group.Enzyme-linked immunosorbent assay(ELISA)was used to detect the expression levels of serum chemerin and PPAR-α.The left atrial diameter(LAD),left ventricular ejection fraction(LVEF),left ventricular end diastolic diameter(LVEDD),and left ventricular mass index(LVMI)were measured;Pearson correlation analysis was used to analyze the correlation between serum chemerin level,PPAR-αlevel and cardiac function indexes;Kaplan-Meier survival curve was used to analyze the relationship between serum chemerin level,PPAR-αlevel and prognosis.Result Compared with healthy group,the expression level of serum chemerin in disease group was significantly up-regulated(P<0.05),the expression level of PPAR-αwas significantly down-regulated(P<0.05);with the increase of New York Heart Association(NYHA)cardiac function grade,the expression level of serum chemerin was significantly increased(all P<0.05),and the expression level of serum PPAR-αwas significantly decreased(all P<0.05).The LAD,LVEDD and LVMI in disease group were significantly higher than those of healthy group(all P<0.05),and the level of LVEF was significantly lower than that of healthy group(P<0.05).Pearson correlation analysis showed that there was a negative correlation between chemerin and PPAR-αexpression levels(r=-0.455,P<0.05);the level of serum chemerin was positively correlated with LAD,LVEDD and LVMI(all P<0.05),and negatively correlated with LVEF(P<0.05),the level of serum PPAR-αwas negatively correlated with LAD,LVEDD and LVMI(all P<0.05),and positively correlated with LVEF(P<0.05).Kaplan-Meier s
