探讨躯体—内脏反射与针刺机制的关系

摘　　要：针刺是传统中医行之有效的治疗方法,但治疗机制尚不清楚。针刺体表产生的酸、麻、胀、痛、木都是躯体感觉信号,借助躯体感觉纤维传入中枢。针刺产生的治疗效应主要包括对心肌、血管平滑肌和内脏平滑肌运动状态的调节,和对内、外分泌腺分泌状态的调节,这些治疗效应恰恰是内脏运动信号借助内脏运动纤维影响效应器的结果。如果来自体表的躯体感觉信号与来自体表或体内病灶的内脏感觉信号传入相同的脑神经节、脊神经节或中枢的感觉核,躯体感觉信号可易化/闭塞内脏感觉神经元,后者产生的内脏感觉信号更容易/更困难地兴奋内脏运动神经元产生治疗效应,故躯体—内脏反射可能是针刺疗法的机制之一。Acupuncture is an effective therapy in traditional Chinese medicine,but the therapeutic mechanism is still unclear.The feelings of sore,tingle,swollen,and pain produced by acupuncture on the body surface are all somatosensory signals,which are transmitted to the nerval center with the help of somatosensory fibers.The therapeutic effects of acupuncture mainly include the regulation of the motor state of cardiac muscle,vascular smooth muscle and visceral smooth muscle,as well as the regulation of the secretion state of internal and exocrine glands.These therapeutic effects are exactly the results of visceral motor signals influencing the effectors with the help of visceral motor fibers.If the signals from body surface of somatosensory and visceral sensory signals from body surface or inside the body lesions passing into the same cerebral ganglion,spinal ganglion or central sensory nuclei,somatosensory signals can easily turn/block visceral sensory neurons,the visceral sensory signals produce by the latter are easier/harder to excite visceral motor neurons to getting therapeutic effects,so the somato-visceral reflex may be one of the mechanisms of acupuncture therapy.
