摘　　要：目的探讨精子顶体酶活性对IVF-ET妊娠结局的影响。方法选取2016年03月–2018年12月于娄底市中心医院符合纳排标准并行新鲜卵裂胚移植的IVF-ET患者共90周期,根据精子顶体酶活性分为实验组(n=24)和对照组(n=66)。分析精子顶体酶活性与OPU日精液常规分析、精子形态学的相关性,比较其对两组精液质量、IVF-ET实验室及临床相关指标的影响。结果两组一般资料比较差异无统计学意义(P>0.05);实验组NSMR、受精率、2PN受精率、着床率、临床妊娠率均低于对照组,精子头部畸形率高于对照组,差异有统计学意义(P<0.05);两组其余各指标比较差异均无统计学意义(P>0.05)。精子顶体酶活性与NSMR、OPU日精液处理后浓度呈正相关(r分别为0.276、0.231,P均<0.05)。结论精子顶体酶活性降低可能会影响IVF-ET卵子受精、降低着床率及临床妊娠率。Objective To investigate the influence of sperm acrosin activity on IVF-ET pregnancy outcome.Methods A total of 90 cycles of IVF-ET patients who met the criteria for inclusion and discharge and transplanted fresh cleavage embryos in Loudi Central Hospital from March 2016 to December 2018 were selected and divided into experimental groups according to sperm acrosin activity(n=24)and the control group(n=66).Analyze the correlation between sperm acrosin activity and routine semen analysis and sperm morphology before(after)OPU daily treatment,and compare its effects on semen quality,IVF-ET laboratory and clinical related indicators of the two groups.Results There was no significant difference in age,BMI,and number of eggs obtained between the two groups of men and women(P>0.05);the percentage of normal sperm morphology,fertilization rate,2PN fertilization rate,implantation rate,and clinical pregnancy rate of the experimental group were lower than those of the control group The rate of sperm head deformity was higher than that of the control group,and the difference was statistically significant(P<0.05);there was no statistically significant difference in the other indicators between the two groups(P>0.05).Sperm acrosin activity was positively correlated with the percentage of normal sperm morphology and the concentration of semen after OPU day treatment(r were 0.276 and 0.231,respectively,P<0.05).Conclusion Decreased sperm acrosin activity may affect IVF-ET egg fertilization,lower implantation rate and clinical pregnancy rate.
