摘 要:目的探讨儿茶酚-O-甲基转移酶(COMT)基因rs4680位点多样性与原发性早泄(LPE)的相关性。方法选取2016年6月至2018年6月丹东市中心医院收治的106例LPE患者作为研究对象(试验组)。另选取同期丹东市中心医院90例健康体检者作为对照组。采用全自动免疫分析仪检测受试者雌二醇(E2)、总睾酮(TT)、游离睾酮(FT)、促黄体生成素(LH)水平,并填写中国早泄患者性功能评价表-5(CIPE-5)。采用Hardy-Weinbery平衡检测平衡程度,通过基质辅助激光解吸-飞行时间质谱仪(MALDI-TOF)检测受试者血清COMT基因rs4680位点基因型及基因分布。比较不同基因型患者E2、TT、FT、LH水平;采用Logistic回归分析COMT基因rs4680位点各基因型与LPE的关系。结果 COMT基因rs4680位点基因型有野生GG型、突变杂合子GA型及突变纯合子AA型。对照组与试验组COMT基因rs4680位点各基因型符合Hardy-Weinberg平衡定律。与对照组比较,试验组E2、TT、FT、LH、GA、AA基因型及A等位基因频率均升高,CIPE-5评分降低,差异具有统计学意义(P<0.05)。试验组野生GG型、杂合子GA型及纯合子突变AA型血清E2、TT、FT、LH水平均依次升高,差异具有统计学意义(P<0.05)。Logistic回归分析显示,COMT基因rs4680位点杂合子GA型、纯合子突变AA型是LPE发生的危险因素(P<0.05)。结论血清中COMT基因rs4680位点GA、AA基因型与LPE关系密切,是发生LPE的危险因素。Objective To investigate the correlation between rs4680 locus diversity of catechol-O-methyltransferase(COMT) gene and lifelong premature ejaculation(LPE). Methods A total of 106 patients with LPE who were treated in Dandong Central Hospital from June 2016 to June 2018 were selected as the study objects(experimental group), and 90 healthy persons in dandong central hospital the same period were selected as the control group. The general data of subjects were collected, the levels of estradiol(E2), total testosterone(TT), free testosterone(FT) and luteinizing hormone(LH) were measured by automatic immune analyzer, and the Chinese index of sexual function forpremature ejaculationg-5(CIPE-5) scale was filled. Hardy-Weinberry equilibrium was used to detect the degree of equilibrium, and matrix assisted laser desorption time of flight mass spectrometer(MALDI-TOF) was used to detect the genotype and gene distribution of rs4680 locus of COMT gene. The levels of E2, TT, FT, LH in different genotypes were compared;and Logistic regression was used to analyze the relationship between rs4680 genotypes of COMT gene and LPE. Results The genotypes of rs4680 locus of COMT gene included wild GG type, mutant heterozygote GA type and mutant homozygote AA type. The genotypes of rs4680 locus of COMT gene in the control group and the experimental group were in accordance with Hardy-Weinberg equilibrium law. Compared with those in the control group, the frequencies of E2, TT, FT, LH, GA, AA genotypes and A allele in the experimental group were higher(P<0.05), and the CIPE-5 score was lower(P<0.05). The levels of E2, TT, FT and LH in wild GG type, heterozygous GA type and homozygous mutation AA type of the patients in the experimental group were all increased in turn(P<0.05). Logistic regression analysis showed that heterozygote GA type and homozygous mutation AA type at rs4680 of COMT gene were risk factors for LPE(P<0.05).
