摘 要:目的探究孕期血清脂蛋白(LP)、p53基因表达水平与小于胎龄儿(SGA)早期生长受限的相关性及临床意义。方法选取2017年6月至2018年12月在湖北省妇幼保健院分娩的200例孕妇及其新生儿作为研究对象。胎儿剪断脐带后即由专业医护人员测量身长、体重,根据《中国不同胎龄新生儿出生体重曲线研制》中体重标准,将其分为SGA组(n=52),适于胎龄儿(AGA)组(n=148)。采用酶联免疫吸附试验(ELISA)法检测血清LP含量,利用自动生化分析仪检测血清学指标,采用荧光定量聚合酶链反应(PCR)技术测定p53相对表达量,采用多因素Logistic回归分析预测影响SGA早期生长受限的危险因素。结果两组孕妇的年龄、产妇类型、新生儿性别比较,差异无统计学意义(P>0.05);SGA组孕周、甘油三酯(TG)水平明显低于AGA组,LP、血清总胆固醇(TC)、高密度脂蛋白胆固醇(HDL-C)、低密度脂蛋白胆固醇(LDL-C)、血清载脂蛋白A1(ApoA1)、载脂蛋白B(ApoB)及p53基因相对表达量均明显高于AGA组,差异具有统计学意义(P<0.05)。多因素Logistic回归分析结果显示,孕周、LP、TC、TG和p53是SGA的危险因素。SGA组新生儿身长、体重均明显低于AGA组,差异具有统计学意义(P<0.05)。孕妇血清LP水平和p53相对表达量水平与新生儿身长、体重均呈负相关(r=-0.402、-0.403,-0.473、-0.456,P<0.05)。结论孕期血清LP水平升高,p53基因相对表达量增加,可能是造成SGA早期生长受限的因素之一。Objective To explore the correlations and clinical significance of serum lipoprotein and p53 gene expression during pregnancy with early growth restriction in small for gestational age infant(SGA). Methods 200 pregnant women and their newborns who delivered in Maternal and Child Health Hospital of Hubei Province from June 2017 to December 2018 were selected as the research subjects. After the umbilical cord was cut, the length and weight of the fetus were measured by professional medical staff. According to the weight standard in the development of birth weight curve of different gestational age newborns in China, 52 newborns were divided into SGA group and 148 suitable for appropriate for gestational age(AGA) group. Enzyme linked immunosorbent assay(ELISA) was used to detect LP in serum, Serological indicators were measured by automatic biochemical analyzer, the relative expression of p53 was determined by fluorescence quantitative PCR, p53 levels and SGA newborn′s length and weight, and multivariate logistic regression analysis was used to predict the risk factors of SGA early growth restriction. Results There was no significant difference in age, type of parturition and sex of newborn between the two groups(P>0.05). Gestational age and triglyceride(TG) levels in SGA group were significantly lower than those in AGA group. The relative expression levels of LP, serum total cholesterol(TC), high density lipoprotein cholesterol(HDL-C), low density lipoprotein cholesterol(LDL-C), serum apolipoprotein A1(ApoA1), apolipoprotein B(ApoB) and p53 genes were significantly higher than those in AGA group(P<0.05). Multivariate Logistic regression analysis showed that gestational age, LP, TC, TG and p53 were risk factors for SGA. Neonatal body length and body weight in SGA group were significantly lower than those in AGA group(P<0.05). The serum LP level and P53 relative expression level of pregnant women were negatively correlated with neonatal body length and weight(r=-0.402,-0.403,-0.473,-0.456, P<0.05). Conclusions The
