宫腔镜下电切术和刨削系统治疗宫内胎物残留的临床效果比较

摘　　要：目的比较宫腔镜下电切术和刨削系统治疗宫内胎物残留的临床效果。方法选取2018年6月至2019年10月惠州市第六人民医院治疗的100例宫内胎物残留患者的临床资料进行回顾性分析。根据最终实施手术方式不同将选择行宫腔镜下电切术治疗的患者作为电切组(n=50),将行宫腔镜下刨削系统治疗的患者作为刨削组(n=50)。对比手术情况、术后恢复情况、一次性治愈率、并发症发生情况。结果刨削组术中出血量明显少于电切组(P<0.05);刨削组术后阴道出血时间、月经恢复正常时间、术后月经量、血绒毛膜促性腺激素(HCG)恢复正常时间及排卵期子宫内膜厚度均明显少于电切组(P<0.05);刨削组与电切组一次性治愈率比较,差异无统计学意义(P>0.05);刨削组并发症总发生率明显低于电切组(P<0.05)。结论刨削组术中出血量、术后阴道出血时间、月经恢复正常时间、血HCG恢复正常时间均明显少于电切组,且并发症少、一次性治愈率高。Objective To compare the clinical effects of hysteroscopic electrotomy and planing system in the treatment of intrauterine fetal remnants. Methods A retrospective analysis of the clinical data of 50 cases of intrauterine fetal remnants treated by hysteroscopic electrotomy(electrotomy group) and 50 cases of intrauterine fetal remnants treated by hysteroscopic planing system(planing group) treated in the Huizhou Sixth People′s Hospital from June 2018 to October 2019. The operation, postoperative recovery, one-time cure rate and complications were compared. Results The amount of bleeding in the planing group was significantly less than that in the electrotomy group(P<0.05). The postoperative vaginal bleeding time, menstrual recovery time, menstrual volume, HCG recovery time and endometrial thickness during ovulation in the planing group were significantly lower than those in the electrotomy group(P<0.05). There was no significant difference in the one-time cure rate between the planing group and the electrotomy group(P>0.05). The total incidence of complications in planing group was significantly lower than that in electrotomy group(P<0.05). Conclusions The intraoperative bleeding volume, postoperative vaginal bleeding time, time of menstrual return to normal, time of blood HCG return to normal in the planing group are significantly less than those in the electrotomy group, and the complications are less and the one-time cure rate is higher.
