摘 要:目的应用超声分层应变成像分析糖尿病兔左心室短轴不同水平段各层心肌的周向应变功能。方法将24只月龄相仿的新西兰兔随机分为两组,每组12例,一组通过耳缘静脉注射四氧嘧啶来诱导糖尿病,作为糖尿病组(n=12);另一组注射等量0.9%氯化钠注射液,作为正常对照组(n=12)。建模成功后第6、9个月时分别对两组兔进行常规超声检查,测量常规参数,采集并存储3个心动周期的二维图像,获取左心室不同短轴及心尖两腔、四腔图像,按17节段划分法脱机分析并计算左心室各短轴水平三层心肌周向应变。结果在第6、9个月时,糖尿病组与正常对照组的常规参数之间比较,差异无统计学意义(P>0.05);在第6个月时,与对照组比较,糖尿病组左心室基底段与心尖段心内膜下心肌周向应变(circumferential strain in endocardium,CSendo)、中层心肌周向应变(circumferential strain in mid-cardium,CSmid)、心外膜下心肌周向应变(circumferential strain in epi-cardium,CSepi)差异均无统计学意义(P>0.05);中间段CSendo减低(P<0.05),而CSmid、CSepi差异无统计学意义(P>0.05);在第9个月时,与对照组比较,糖尿病组心尖段、基底段CSendo、CSmid、CSepi差异均无统计学意义(P>0.05),中间段CSendo、CSmid、CSepi均减低(P<0.05)。结论分层应变技术可以早期发现糖尿病兔左心室心肌功能异常,准确评价左心室短轴不同水平段的各层心肌功能。Objective To analyze the circumferential strain of different layers of left ventricular short axis in diabetic rabbits by using ultrasoniclayer-specific strain,so as to evaluate the effect of diabetes on left ventricular myocardial function.Methods A total of twenty-four New Zealand rabbits of similar age were randomly divided into two groups,each with 12 cases.One group was treated as a diabetic group by injecting alloxan into the ear vein(n=12).The other group was injected with the same amount of normal saline as normal control group(n=12).At the 6th and 9th months after the successful modeling,the two groups of rabbits were examined to routine ultrasound examinations,routine parameters were measured,and two-dimensional images of 3 cardiac cycles were collected to obtain the three horizontal segments of the left ventricular short axis and the apical two-chamber and four-chamber images,the images were stored and analyzed offline by workstation.According to the 17-segment division method,the three-layer myocardium of each short axis of left ventricle was analyzed,and the circumferential strain of each layer of left ventricular myocardium was calculated.Results(1)At the 6th and 9th month,there was no significant difference in routine parameters between the diabetic group and the normal control group(P>0.05).(2)At the 6th month,compared with the control group,there was no significant difference in subendocardial circumferential strain(circumferential strain in endocardium,CSendo),middle myocardial circumferential strain(circumferential strain in mid-cardium,CSmid)and epicardial circumferential strain(circumferential strain in epi-cardium,CSepi)between the basal and apical segments of the left ventricle in the diabetic group(P>0.05),CSendo decreased in the middle segment(P<0.05),while CSmid and CSepi were not statistically significant(P>0.05).At the 9th month,compared with the control group,there was no significant difference in CSendo,CSmid and CSepi in the apical segmentand the basal segment(P>0.05),while CSendo,C
