摘　　要：目的:探讨完全性左束支传导阻滞(CLBBB)合并左室收缩功能下降患者的心电图特点,提高心电图对精细化CLBBB诊断标准的作用。方法:收集2018年1月至2019年3月就诊于我院的CLBBB患者191例,根据LVEF大小,患者被分为LVEF≥50%组(95例)和LVEF<50%组(96例)。测量比较两组一般资料、心电图指标。结果:与LVEF≥50%组比较,LVEF<50%组左室舒张末内径(LVEDd)[(5.0±0.4)cm比(6.3±0.9)cm]、左房内径(LAD)[(3.8±0.4)cm比(4.4±0.6)cm]、QRS时限[(143.7±10.9)ms比(156.5±19.6)ms]、校正的QT间期(QTc)[(470.7±29.7)ms比(488.3±37.5)ms]、电轴左偏比例(26.3%比40.6%)、电轴左偏度数[-11.000(-36.000~19.000)比-20.000(-40.750~2.000)]均显著增大,R v1振幅[0.150(0.030~0.290)比0.080(0.023~0.200)]显著减小,P<0.05或<0.01。结论:CLBBB合并左室收缩功能下降的患者QRS时限、QTc间期显著延长,电轴偏移显著,R V1振幅显著降低。Objective:To explore ECG characteristics of patients with complete left bundle branch block(CLBBB)complicated decreased left ventricular systolic function(dLVSF),and improve role of ECG in refining diagnostic standard of CLBBB.Methods:A total of 191 CLBBB patients treated in our hospital from Jan 2018 to Mar 2019 were collected,according to LVEF,patients were divided into LVEF≥50%group(n=95)and LVEF<50%group(n=96).General data and ECG indexes were measured and compared between two groups.Results:Compared with LVEF≥50%group,there were significant rise in left ventricular end-diastolic dimension(LVEDd)[(5.0±0.4)cm vs.(6.3±0.9)cm],left atrial diameter(LAD)[(3.8±0.4)cm vs.(4.4±0.6)cm],QRS duration[(143.7±10.9)ms vs.(156.5±19.6)ms],corrected QT interval(QTc)[(470.7±29.7)ms vs.(488.3±37.5)ms],percentage of left electric axis(26.3%vs.40.6%),left skewed electric axis angle[-11.000(-36.000~19.000)vs.-20.000(-40.750~2.000)],and significant reduction in R v1 amplitude[0.150(0.030~0.290)vs.0.080(0.023~0.200)]in LVEF<50%group,P<0.05 or<0.01.Conclusion:In CLBBB patients with decreased left ventricular systolic function,there are significant rise in QRS duration and QTc,significantly skewed electric axis,and significant reduction in R V1 amplitude.
