摘　　要：目的:探讨清热凉血中药煎剂含漱联合牙周基础治疗对种植牙患者牙周指标、白细胞介素1β(IL-1β)、肿瘤坏死因子α(TNF-α)及基质金属蛋白酶2(MMP-2)水平的影响。方法:将2018年10月至2020年4月唐山市协和医院收治的牙列缺损患者76例按随机数表法设置为研究组和对照组,每组38例。两组患者均行即刻修复种植术,术后对照组患者给予西医基础治疗,研究组患者在对照组的基础上加用含漱清热凉血中药煎剂,均于术后1个月回院复查。治疗前及治疗后1个月以视诊加探诊方式检测两组患者龈沟出血指数(SBI)、牙龈指数(GI)、菌斑指数(PLI)以及龈沟探诊深度(SPD),同时检测两组患者龈沟液内IL-1β、TNF-α及MMP-2含量。结果:治疗前,两组患者SBI、GI、PLI及SPD的差异均无统计学意义(P>0.05);治疗后,研究组患者SBI、GI、PLI及SPD明显低于对照组,差异均有统计学意义(P<0.05)。治疗前,两组患者龈沟液IL-1β、TNF-α及MMP-2含量的差异均无统计学意义(P>0.05);治疗后,研究组患者龈沟液IL-1β、TNF-α及MMP-2含量明显低于对照组,差异均有统计学意义(P<0.05)。结论:在牙周常规治疗基础上联合应用清热凉血中药煎剂含漱能够有效改善种植牙患者的口腔炎症,提高牙周健康程度,并降低龈沟液内IL-1β、TNF-α及MMP-2等细胞因子的含量。OBJECTIVE:To probe into the effects of periodontal basic treatment combined with heat-clearing and blood-cooling traditional Chinese medicine decoction on periodontal indexes,interleukin-1β(IL-1β),tumor necrosis factorα(TNF-α)and matrix metalloproteinase-2(MMP-2)levels in patients with dental implants.METHODS:Totally 76 patients with dentition defect admitted into Tangshan Union Hospital from Oct.2018 to Apr.2020 were extracted to be divided into the research group and the control group via the random number table,with 38 cases in each group.Both groups received immediate restorative implant surgery.After surgery,the control group was given basic western medicine treatment,while the research group was treated with heat-clearing and blood-cooling traditional Chinese medicine decoction on the basis of the control group.Patients in both groups returned to the hospital for follow-up at 1 month after surgery.Before treatment and 1 month after treatment,the gingival sulcus bleeding index(SBI),gingival index(GI),plaque index(PLI)and sulcus probing depth(SPD)of two groups were detected by inspection and exploration.The levels of IL-1β,TNF-αand MMP-2 in gingival crevicular fluid of both groups were detected.RESULTS:Before treatment,there was no difference in SBI,GI,PLI and SPD between two groups(P>0.05).After treatment,SBI,GI,PLI and SPD in the research group were significantly lower than those in the control group,with statistically significant difference(P<0.05).Before treatment,there was no significant difference in the levels of IL-1β,TNF-αand MMP-2 in gingival crevicular fluid between two groups(P>0.05).After treatment,the levels of IL-1β,TNF-αand MMP-2 in the gingival crevicular fluid of the research group were significantly lower than those of the control group,and the differences were statistically significant(P<0.05).CONCLUSIONS:Periodontal basic treatment combined with heat-clearing and blood-cooling traditional Chinese medicine decoction can effectively improve the oral inflammation,promote the period
