摘　　要：目的:了解首都医科大学附属北京友谊医院(以下简称“我院”)中药饮片处方情况,促进临床合理用药,保障患者用药安全,降低临床不合理用药率。方法:从2020年1—9月我院23188张门诊中药饮片处方中随机抽取3000张进行点评与分析,并进行分类汇总、统计分析。结果:3000张门诊中药饮片处方中,不合理处方252张,占8.40%。其中,未按要求标记特殊药物调剂、煎煮方法的处方有131张,占不合格处方的51.98%,占抽取总处方数的4.37%;用法、用量不适宜处方有109张,占不合格处方的43.25%,占抽取总处方数的3.63%。常用的有小毒及有毒中药饮片超量使用涉及吴茱萸、水蛭、苦杏仁、蒺藜、川楝子和黑附片,剂量超出率范围在20%~233.3%。结论:我院门诊中药饮片处方仍存在一定的不合理现象,通过处方点评,药师可在工作中及时发现问题,与医师反馈问题,帮患者处理问题,确保临床用药安全。OBJECTIVE:To investigate the current status of outpatient prescriptions of traditional Chinese medicine decoction pieces in Beijing Friendship Hospital,Capital Medical University(hereinafter referred to as“our hospital”),to promote rational clinical drug use,so as to ensure the drug safety of patients and reduce the irrational rate of clinical drug use.METHODS:From Jan.to Sept.in 2020,3000 prescriptions of traditional Chinese medicine decoction pieces were randomly selected from 23188 outpatient prescriptions of our hospital for review and analysis,classification and statistical analysis.RESULTS:Among the 3000 outpatient prescriptions of traditional Chinese medicine decoction pieces,252 prescriptions were irrational,accounting for 8.40%.There were 131 prescriptions that did not mark the dispensing and decoction methods of special drugs as required,accounting for 51.98%of the unqualified prescriptions and 4.37%of the total extracted prescriptions.There were 109 prescriptions with irrational usage and dosage,accounting for 43.25%of the unqualified prescriptions and 3.63%of the total extracted prescriptions.The commonly used off-amount use of small poison and poisonous traditional Chinese medicine decoction pieces included fructus evodiae,hirudo,semen armeniacae amarum,tribulus terrestris,melia toosendan and Acanthophora chinensis tablets,and the off-amount use rate ranged from 20%to 233.3%.CONCLUSIONS:There are still some irrational phenomena in the prescriptions of traditional Chinese medicine decoction pieces in outpatient department of our hospital.Through prescription review,pharmacists can timely find problems in clinic,feedback problems with clinicians,help patients deal with the problems,and ensure the safety of clinical medication.
