桂枝茯苓丸联合超声引导下聚桂醇治疗盆腔囊肿的临床研究

摘　　要：目的探讨桂枝茯苓丸联合超声引导下聚桂醇治疗盆腔囊肿的疗效。方法选取2017年12月至2020年7月期间在益阳市中心医院行超声引导下聚桂醇硬化介入治疗的盆腔囊肿患者54例,随机分为观察组和对照组,各27例,对照组采用超声引导下聚桂醇硬化治疗,观察组在对照组基础上口服桂枝茯苓丸治疗。治疗1个月、3个月、6个月后在超声下观察囊肿最大直径,检测血清IL-6、IL-8、ICAM-1的表达,并比较两组的有效率及复发率。结果两组治疗后各时间点的囊肿最大直径均小于治疗前(P<0.05),且观察组囊肿最大直径均小于对照组(P<0.05);与对照组相比,观察组各时间点患者血清炎症相关因子IL-6、IL-8、ICAM-1含量均下降(P<0.05);治疗后3个月,观察组有效率明显高于对照组(P<0.05);随访6个月,观察组复发率明显低于对照组(P<0.05)。结论桂枝茯苓丸联合超声引导下聚桂醇治疗盆腔囊肿能够降低复发率、提高疗效,可能与其降低血清IL-6、IL-8、ICAM-1相关,值得临床推广应用。Objective To explore the efficacy of Guizhi Fuling Pills combined with lauromacrogol under ultrasound guidance in the treatment of pelvic cysts.Methods 54 patients with pelvic cysts who underwent ultrasound-guided interventional therapy of lauromacrogol sclerotherapy in Yiyang Central Hospital from December 2017 to July 2020 were randomly divided into observation group and control group,with 27 cases in each group,and the control group was guided by ultrasound under lauromacrogol sclerotherapy,the observation group was treated with Guizhi Fuling Pills on the basis of the control group.After treatment for 1 month,3 months,and 6 months,the diameter of the cyst was observed under ultrasound,and the expression of serum IL-6,IL-8,ICAM-1 was detected.The effective rate and recurrence rate of the two groups were compared.Results The maximum diameter of the cyst at each time point after treatment in the two groups was smaller than before treatment(P<0.05),and the maximum diameter of the cyst in the observation group was smaller than the control group(P<0.05).Compared with the control group,the serum inflammation-related factors IL-6,IL-8 and ICAM-1 levels in the observation group decreased at each time point(P<0.05).3 months after treatment,the effective rate of the observation group was significantly higher than that of the control group(P<0.05);after follow-up for 6 months,the recurrence rate of the observation group was significantly lower than that of the control group(P<0.05).Conclusion Guizhi Fuling Pills combined with ultrasound-guided lauromacrogol in the treatment of pelvic cysts has the advantages of reducing the recurrence rate and improving the curative effect,which may be related to the reduction of serum IL-6,IL-8,ICAM-1,and it is worthy of clinical application.
