摘 要:目的:探讨临床医师情感耗竭与家庭-工作冲突之间的关系。方法:2010年共招募临床医师2530名,采用自编问卷收集社会人口学和工作相关资料,采用中文版Maslach职业倦怠量表(CMBI)的情感耗竭维度(EE)、工作影响家庭和家庭影响工作量表(WFCS)的家庭-工作冲突维度(FWC)、简易应对方式问卷(SCSQ)评估情感耗竭、家庭-工作冲突及应对方式。结果:多因素线性回归分析显示,EE得分与FWC得分正向关联(β=0.39,95%CI:0.33~0.46)。广义相加混合模型曲线拟合结果显示,EE得分与FWC得分之间为“J”型非线性关系,存在1个拐点。以拐点分段进行多因素线性回归分析显示,当FWC得分<6.0时,EE得分随FWC得分升高而降低(β=-1.82,95%CI:-3.02~-0.63);当FWC得分≥6.0时,EE得分随FWC得分升高而升高(β=0.41,95%CI:0.34~0.48)。结论:本研究显示,中国临床医师的情感耗竭水平与家庭-工作冲突呈非线性关系,过高或过低水平的家庭-工作冲突可能对情感耗竭有不利影响。Objective:To investigate the relationship between burnout-related emotional exhaustion(EE)and family-work conflict(FWC)in physicians.Methods:Totally 2530 physicians were recruited in a cross-sectional study in 2010.Data pertaining to socio-demographic and work related characteristics were collected.The EE,FWC and coping styles of all participants were assessed with the Chinese Maslach Burnout Inventory Questionnaire(CM-BI),Work Family Conflict Scale(WFCS)and Simplified Coping Style Questionnaire(SCSQ).Results:A multivariate regression analysis showed a significant positive correlation between EE scores and FWC scores(β=0.39,95%CI:0.33-0.46,P<0.001).The curve fitting results of generalized additive mixed model showed that there were one inflexion points between EE scores and FWC scores.In multivariate piecewise linear regression,the levels of EE score decreased with the elevation of FWC scores when FWC scores were less than 6.0(β=-1.82,95%CI:-3.02 to-0.63,P=0.003).The levels of EE scores increased with the elevation of FWC scores when FWC scores were greater than 6.0(β=0.41,95%Cl:0.34-0.48,P<0.001).Conclusion:This study demonstrated a nonlinear relationship between emotional exhaustion and family-work conflict level sin physicians,and high or low levels of family-work conflict may have an adverse effect on emotional exhaustion.
