硝普钠联合多巴酚丁胺治疗冠心病合并心力衰竭的疗效分析

摘　　要：目的分析硝普钠联合多巴酚丁胺应用于冠心病合并心力衰竭治疗中的疗效。方法108例冠心病合并心力衰竭患者,按照电脑随机法分为对照组和实验组,各54例。对照组采用多巴酚丁胺治疗,实验组采用硝普钠联合多巴酚丁胺治疗。对比两组治疗满意率、治疗效果、治疗前后心功能指标。结果实验组治疗总满意率为94.44%,高于对照组的81.48%,差异有统计学意义(P<0.05)。实验组治疗总有效率为96.30%,高于对照组的85.19%,差异有统计学意义(P<0.05)。治疗后,实验组左心室收缩末期内径(LVESD)、左心室射血分数(LVEF)、心功能评分均优于治疗前,且实验组LVESD(36.52±4.22)mm、LVEF(46.25±2.31)%、心功能评分(88.22±3.43)分优于对照组的(45.52±5.86)mm、(35.25±3.30)%、(56.00±4.52)分,差异有统计学意义(P<0.05)。两组治疗前后左心室舒张末期内径(LVEDD)对比差异无统计学意义(P>0.05)。结论硝普钠联合多巴酚丁胺治疗冠心病合并心力衰竭效果显著,可提升患者的治疗满意率,改善心功能,临床应用价值明显,建议推广。Objective To analyze the efficacy of sodium nitroprusside combined with dobutamine in the treatment of coronary heart disease complicated with heart failure.Methods A total of 108 cases of coronary heart disease complicated with heart failure were divided into control group and experimental group according to computer random grouping method,with 54 cases in each group.The control group was treated with dobutamine monotherapy,and the experimental group was treated with sodium nitroprusside combined with dobutamine.The treatment satisfaction rate,therapeutic effect,and cardiac function indicators before and after treatment were compared between the two groups.Results The total satisfaction rate of the experimental group was 94.44%,which was higher than 81.48%of the control group,and the difference was statistically significant(P<0.05).The total effective rate 96.30%of the experimental group was higher than 85.19%of the control group,and the difference was statistically significant(P<0.05).After treatment,the left ventricular end systolic diameter(LVESD),left ventricular ejection fraction(LVEF)and cardiac function score of the two groups were better than those before intervention;the LVESD(36.52±4.22)mm,LVEF(46.25±2.31)%and cardiac function score(88.22±3.43)points of the experimental group were better than(45.52±5.86)mm,(35.25±3.30)%and(56.00±4.52)points of the control group.All the difference was statistically significant(P<0.05).There was no statistically significant difference in left ventricular end diastolic diameter(LVEDD)between the two groups before and after intervention(P>0.05).Conclusion Combination of sodium nitroprusside and dobutamine has remarkable effect on coronary heart disease complicated with heart failure,which can increase the satisfaction rate of patients with treatment,improve cardiac function,and have obvious clinical value.It is recommended to be promoted.
