摘 要:目的分析新生儿持续肺动脉高压(PPHN)患儿采取一氧化氮吸入(iNO)联合高频震荡通气(HFOV)治疗的效果。方法55例PPHN患儿,随机分为对照组(27例)与观察组(28例)。对照组予以HFOV治疗,观察组在对照组基础上予以iNO治疗。对比两组患儿治疗前后氧合指数及肺动脉压水平,氧疗时间、呼吸机治疗时间及住院治疗时间。结果治疗后,观察组患儿肺动脉压及氧合指数分别为(15.26±3.87)、(8.39±4.12)mm Hg(1 mm Hg=0.133 kPa),均显著低于对照组的(29.45±3.28)、(13.78±3.76)mm Hg,差异均具有统计学意义(P<0.05)。观察组患儿氧疗时间、呼吸机治疗时间及住院治疗时间均短于对照组,差异有统计学意义(P<0.05)。结论PPHN患儿采取iNO及HFOV联合治疗方案,治疗效果优于单独应用HFOV治疗,可缩短患儿呼吸机治疗、氧疗和住院治疗时间,尽快降低肺动脉压提升氧合指数,临床价值显著值得推广应用。Objective To analyze the effect of inhaled nitric oxide(iNO)combined with high frequency oscillatory ventilation(HFOV)in persistent pulmonary hypertension of newborns(PPHN).Methods A total of 55 cases of PPHN children were randomly divided into control group(27 cases)and observation group(28 cases).The control group was HFOV,and the observation group was treated with iNO based on the control group.The oxygenation index and pulmonary artery pressure before and after treatment,oxygen therapy time,ventilator treatment time and hospitalization time were compared between the two groups.Results After treatment,the pulmonary artery pressure and oxygenation index of the observation group were(15.26±3.87)and(8.39±4.12)mm Hg(1 mm Hg=0.133 kPa),which were significantly lower than(29.45±3.28)and(13.78±3.76)mm Hg of the control group,and the difference was statistically significant(P<0.05).The oxygen therapy time,ventilator treatment time and hospitalization time of the observation group were shorter than those of the control group,and the difference was statistically significant(P<0.05).Conclusion The combined treatment of INO and HFOV in PPHN is better than that of HFOV alone.It can shorten the ventilator treatment time,oxygen therapy time and hospitalization time,reduce pulmonary artery pressure and improve oxygenation index as soon as possible.It has significant clinical value,and is worthy of popularization and application.
