摘　　要：目的分析白细胞介素(IL)-8和核因子κB1(NF-κB1)的基因多态性与2型糖尿病(T2DM)并发幽门螺杆菌(Hp)感染患者胃溃疡易感性的关系.方法选取2017年8月-2020年8月焦作市人民医院内分泌科收治的93例T2DM并发Hp感染患者,根据患者是否发生胃溃疡分为胃溃疡组32例和无溃疡组61例.检测所有患者白细胞计数(WBC)、血清肿瘤坏死因子-α(TNF-α)、IL-6和IL-8水平,PCR检测IL-8基因rs4073(A/T)、rs2227306(C/T)和NF-κB1基因rs3774963(C/G)、rs28362491(-94ins/delATTG)位点多态性.结果胃溃疡组患者IL-8基因位点rs4073携带等位基因T的频率明显高于无溃疡组(P<0.05),NF-κB1基因位点rs3774963携带等位基因G的频率明显高于无溃疡组(P<0.05),rs28362491位点-94insATTG的频率明显高于无溃疡组(P<0.05),且血清TNF-α、IL-6、IL-8水平明显高于无溃疡组(P<0.05).结论IL-8基因rs4073位点等位基因T、NF-κB1基因rs3774963位点等位基因G和rs28362491位点-94insATTG可以增加T2DM并发Hp感染患者胃溃疡的易感性.OBJECTIVE To analyze the relationship between interleukin-8(IL-8)and gene polymorphism of nuclear factorκB1(NF-κB1)and the susceptibility to gastric ulcer in the type 2 diabetes mellitus(T2 DM)patients complicated with Helicobacter pylori(Hp)infection.METHODS A total of 93 T2 DM patients complicated with Hp infection who were treated in the department of endocrinology of Jiaozuo People′s Hospital from Aug 2017 to Aug 2020 were enrolled in the study and were divided into the gastric ulcer group with 32 cases and the no gastric ulcer group with 61 cases according to the status of gastric ulcer.The levels of white blood cell(WBC),serum tumor necrosis factor-α(TNF-α),IL-6 and IL-8 of all the patients were detected,and the polymorphisms at IL-8 gene at rs4073(A/T)and rs2227306(C/T)loci,and NF-κB1 gene at rs3774963(C/G)and rs28362491(-94 ins/delATTG)loci were detected by PCR.RESULTS The frequency of allele T in IL-8 gene at rs4073 locus was significantly higher in the gastric ulcer group than in the non gastric ulcer group(P<0.05),the frequency of allele G in NF-κB1 gene at rs3774963 locus was significantly higher in the gastric ulcer group than in the no gastric ulcer group(P<0.05),the frequency of-94 insATTG at rs28362491 locus was significantly higher in the gastric ulcer group than in the no gastric ulcer group(P<0.05).The levels of serum TNF-α,IL-6 and IL-8 of the gastric ulcer group were significantly higher than those of the no gastric ulcer group(P<0.05).CONCLUSION The allele T of IL-8 gene at rs4073 locus,allele G at rs3774963 locus and-94 insATTG at rs28362491 locus of NF-κB1 gene may increase the susceptibility to gastric ulcer in the T2 DM patients complicated with Hp infection.
