摘　　要：目的探究幽门螺杆菌(Hp)合并下呼吸道感染患者血沉(ESR)、血清可溶性髓系细胞触发受体-1(sTREM-1)、转化生长因子β1(TGF-β1)变化及对治疗结局的影响。方法选取天津医科大学总医院消化内科2017年8月-2019年9月收治的Hp合并下呼吸道感染患者72例作为研究组,单纯Hp阳性患者72例作为单纯Hp阳性组,Hp阴性、下呼吸道感染患者72例作为单纯下呼吸道感染组,另选择同期健康体检人群72例为健康对照组。比较4组ESR、血清sTREM-1、TGF-β1水平差异,评价其鉴别诊断价值。研究组根据治疗结局分为治愈患者与未治愈患者,分析上述指标与治疗结局的关联性。结果入组4组研究对象ESR、sTREM-1、TGF-β1水平比较差异有统计学意义(P<0.05);研究组治疗前ESR、血清sTREM-1、TGF-β1水平高于单纯下呼吸道感染组、单纯Hp阳性组、健康对照组,单纯下呼吸道感染组高于单纯Hp阳性组、健康对照组,单纯Hp阳性组高于健康对照组(P<0.05);受试者工作特征曲线(ROC)结果显示,血清sTREM-1诊断的Hp合并下呼吸道感染、单纯下呼吸道感染及单纯Hp阳性的曲线下面积(AUC)均>0.800,高于ESR、TGF-β1;研究组不同治疗结局与患者下呼吸道感染类型、合并糖尿病、慢性阻塞性肺疾病(COPD)比例、治疗方案有关(P<0.05);研究组内治愈患者ESR、血清sTREM-1、TGF-β1水平均低于未治愈患者(P<0.05)。结论Hp合并下呼吸道感染患者ESR、血清sTREM-1、TGF-β1水平显著升高,并与治疗结局显著相关,早期检测各指标水平可为Hp合并下呼吸道感染鉴别、实施针对性治疗方案提供新思路。OBJECTIVE To explore the changes of erythrocyte sedimentation rate(ESR),serum soluble triggering receptor expressed on myeloid cells-1(sTREM-1)and transforming growth factor-β1(TGF-β1)in the Helicobacter pylori(HP)infection patients complicated with lower respiratory tract infection and observe the effect on treatment outcomes.METHODS Totally 72 Hp infection patients complicated with lower respiratory tract infection who were treated in the gastroenterology department of the General Hospital of Tianjin Medical University from Aug.2017 to Sep.2019 were assigned as the study group,72 simple Hp-positive patients were regarded as the simple Hp-positive group,72 Hp-negative patients with lower respiratory tract infection were regarded as the simple lower respiratory tract infection group,meanwhile,72 healthy people who received physical examination were set as the control group.The levels of ESR,serum sTREM-1 and TGF-β1 were compared among the four groups,and the differential diagnosis values of the indexes were evaluated.The patients were divided into the cured group and the uncured group according to the treatment outcome,and the correlation between above indexes and the treatment outcome was observed.RESULTS There were significant differences in the levels of ESR,serum sTREM-1 and TGF-β1 among the four groups of study objects(P<0.05).The levels of ESR,serum sTREM-1 and TGF-β1 of the study group were significantly higher than those of the simple lower respiratory tract infection group,the simple Hp-positive group and the control group;the levels of above indexes were significantly higher in the simple lower respiratory tract infection group than in the simple Hp-positive group and the control group,and the levels of above indexes were significantly higher in the simple Hp-positive group than in the control group(P<0.05).ROC curve analysis showed that the area under curve(AUC)of the serum sTREM-1 was more than 0.800 in diagnosis of Hp infection complicated with lower respiratory tract infection,simple lower resp
