摘 要:目的探讨脑梗死患者感染风险与CD4^(+)T细胞及ZNRD1基因多态性的相关性。方法选取2015年1月-2019年12月南阳医学高等专科学校第一附属医院神经内科收治的200例脑梗死患者为研究对象。依据是否感染分为感染组(86例)和非感染组(114例)。采用流式细胞仪检测CD4^(+)T细胞水平,应用PCR及限制性内切酶酶切技术检测ZNRD1基因多态性,并分析脑梗死患者感染风险与CD4^(+)T细胞及ZNRD1基因多态性的相关性。结果感染组患者性别、年龄、BMI等基线资料与非感染组比较差异无统计学意义;感染组患者CD4^(+)T细胞水平低于非感染组(P<0.05);感染组ZNRD1基因rs 16896970位点基因型为AA的分布频率和等位基因为A的分布频率均高于非感染组(P<0.05);ZNRD1基因rs 3869068、rs 1048412位点,显性模型及隐性模型均未发现与脑梗死感染发病风险相关,而rs 16896970位点显性模型与脑梗死感染发病风险相关(P<0.05)。结论ZNRD1 rs 16896970位点突变纯合子AA及等位基因A和CD4^(+)T细胞水平与脑梗死后感染发生密切相关,有望为脑梗死后感染的诊断或预防提供重要参考信息。OBJECTIVE To explore the correlation between CD_(4)^(+)T cell,ZNRD1 gene polymorphism and the risk of infection in patients with cerebral infarction.METHODS A total of 200 patients with cerebral infarction who were treated in the National Education College,Nanyang Medical College from Jan.2015 to Dec.2019 were recruited as the study subjects and divided into the infection group with 86 cases and the non-infection group with 114 cases according to the status of infection.The level of CD_(4)^(+)cells was detected by using flow cytometry,the ZNRD1 gene polymorphism was determined with the use of PCR and restriction enzyme digestion technique,and the correlation between the CD_(4)^(+)T cell,ZNRD1 gene polymorphism and the risk of infection was observed.RESULTS There were no significant differences in the gender,age and BMI between the infection group and the non-infection group;the CD_(4)^(+)T level of the infection group was significantly lower than that of the non-infection group(P<0.05).The distribution frequencies of genotype AA and allele A of ZNRD1 rs 16896970 of the infection group were significantly higher than those of the non-infection(P<0.05).The risk of infection in the cerebral infarction patients was not found to be related to the ZNRD1 gene rs 3869068,rs 1048412 l loci,dominance model and recessiveness model,while the dominance model of rs 16896970 locus was significantly associated with the risk of infection in the cerebral infarction patients(P<0.05).CONCLUSION The mutant homozygous AA and allele A of ZNRD1 rSl6896970 locus and levels of CD_(4)^(+)T cells are closely associated with the infection in the cerebral infarction patients,and the above indexes are expected to provide important reference for diagnosis or prevention of infection in the cerebral infarction patients.
