摘 要:目的研制一种充气式耳鼻异物取出器并分析临床试用效果。方法采用混合聚乙烯与聚苯二甲酸乙二醇脂为材料,设计充气式耳鼻异物取出器,主要由充气装置、异物取出装置组成(弹性气囊在充气管顶端,有伞型和钩型两组)。选择2018年1月1日至12月31日在中国人民解放军陆军第七十三集团军医院眼耳鼻喉科门诊就诊鼻内异物患者140例,其中男性87例,女性53例;年龄2~65岁,中位年龄28岁。随机数字表法分为对照组和试验组,每组70例。对照组采用传统的耵聍钩或者异物钳取出耳鼻异物,试验组采用充气式耳鼻异物取出器取出耳鼻异物。比较两组患者异物取出的时间、成功率,并对医务人员和患者在诊疗过程中的感受进行调查和评估。结果伞型和钩型取出装置采用充气式弹性球囊,不会刮伤耳鼻腔内组织,安全可靠。对照组异物取出时间为(31.00±2.57) min,试验组为(22.00±2.32) min;两组比较,差异有统计学意义(χ^(2)=19.321,P <0.05)。对照组异物取出成功率68.57%,低于试验组(87.14%);两组比较,差异有统计学意义(χ^(2)=21.265,P <0.05)。患者采用充气式耳鼻异物取出器的满意度91.43%,高于对照组(50.00%);两组比较,差异有显著统计学意义(χ^(2)=41.682,P <0.01)。医务人员采用充气式耳鼻异物取出器的满意度86.43%,高于对照组(62.86%);两组比较,差异有显著统计学意义(χ^(2)=20.549,P <0.01)。结论设计的充气式耳鼻异物取出器操作简单,实用性强,提高了异物取出成功率,减少患者诊治时间,增加医务人员和患者诊疗过程中的舒适感,值得临床使用。Objective To analyze clinical effect of the developed inflatable ear-nose protractor. Methods The polyethylene and ethylene glycol polyphenylene dicarboxylate were used to design inflatable ear-nose protractor which was mainly composed of inflatable device and protractor(umbrella-shaped and hook-shaped elastic airbags on top of inflatable tube). From January 1 to December 31 in 2018, 140 patients with intranasal foreign bodies were enrolled, which included 87 males and 53 females, aged 2-65 years old with medium age of 28 years old. According to random number table method, patients were divided into control group(n = 70, with traditional hook or forceps) and test group(n = 70, with inflatable ear-nose protractor).The time and success rate of foreign body removal between 2 groups were compared, and the feelings of medical staff and patients during diagnosis and treatment were investigated and evaluated. Results The umbrella-shaped and hook-shaped inflatable elastic balloons were safe and reliable without scratching tissues in ear and nose. The foreign body removal time of test group(22.00 minutes ± 2.32 minutes) was statistically significantly less than control group(31.00 minutes ± 2.57 minutes)(χ^(2)=19.321, P < 0.05). The success rate of foreign body removal in test group(87.14 %) was statistically significantly higher than in control group(68.57 %)(χ^(2)= 21.265, P < 0.05). The satisfaction rate of inflatable ear-nose protractor(91.43 %) was statistically significantly higher than that of control group(50.00 %)(χ^(2)= 41.682, P < 0.01). The medical staff satisfaction in test group(86.43 %)was statistically significantly higher than that of control group(62.86 %)(χ^(2)= 20.549, P < 0.01). Conclusion It is demonstrated that inflatable ear-nose protractor is simple and practicability. It improves success rate of foreign body removal, reduces diagnosis and treatment time, and increases comfort in diagnosis and treatment, so it is worthy of clinical application.
