完整系膜游离在腹腔镜阑尾切除术中的应用价值

摘　　要：[目的]探讨完整阑尾系膜游离技术在腹腔镜下阑尾切除术中的应用价值。[方法]选择2016年7月至2018年7月安徽省亳州市人民医院收治的450例行腹腔镜下阑尾切除术的急性阑尾炎患者资料,所有患者均采用完整阑尾系膜游离的方法,分析手术时间、术中出血、术后并发症、住院时间。[结果]450例患者均完成腹腔镜下阑尾切除术,采用的是完整系膜游离保留阑尾系膜的方式。其中行顺行阑尾切除术416例,逆行阑尾切除术34例;手术时间25~90(36±3.2)min;术中出血0~10(5.2±1.1)mL;术后6~24 h进食流质,术后住院时间3~6(3.3±0.9)d.术后出现Trocar孔感染6例,经临床抗感染治疗均恢复,无残端瘘、腹盆腔脓肿形成、术后出血等并发症发生。术后随访1~3年,均无粘连性肠梗阻等远期并发症发生。[结论]完整系膜游离技术是一种安全、有效的腹腔镜下阑尾切除技术,可推广应用于临床治疗急性阑尾炎。[Objective]To investigate the technique of intact mesenteric separation in laparoscopie appen-dectomy.[Methods]The data of 450 patients undergoing laparoscopic appendectomy from July 2016 to July 2018 were collected in our hospital,to summarize and analyze the operation time,intraoperative bleeding,postoperative complications,and hospital stays.[Results]All 450 patients successfully were completed by lap-aroscopic appendectomy under the intact mesangial separation technique.The operation time was 25-90min,with an average of 35min.The intraoperative bleeding was 0-10mL.The hospitalization time was 3-6 days.There were 6 cases of postoperative wound infection,and no stump fistula,abdominal abscess,postoperative bleeding and other complications occurred,the patients were followed up for 1-3 years,and no adhesive ileus occurred.[Conclusion]The intact mesentery separation technique is safe and effective,which can be applied in clinical practice.
