基于三级综合医院等级评审结果的临床检验管理现状分析

摘　　要：目的分析三级综合医院临床检验管理存在的共性问题并探讨解决对策,为医院临床实验室完善质量管理和加强持续改进提供数据支持和参考依据。方法对我国151所三级综合医院等级评审数据中“临床检验管理与持续改进条款”部分进行归纳分析。结果临床实验室制度建设、人员、试剂与校准品管理、实验室信息管理系统、24小时急诊检验服务、危急值报告等方面比较规范,持续改进效果明显;但在开展新项目的管理和审批、临床小实验室管理、POCT项目管理、实验室安全以及质量控制等方面存在一些比较突出和普遍的问题,还有较大的改进空间。结论三级综合医院临床实验室应聚焦共性问题和薄弱环节,科学运用管理工具进行系统分析和追踪监控,持续改进临床检验管理水平。Objective To analyze the pervasive problems in clinical laboratory management and discuss their feasible solutions to assist evidence-based policy making in terms of enhancing clinical laboratory management and continuous quality improvement.Methods Systematically analyzing accreditation results of"Clinical Laboratory Management and Continuous Improvement Clauses"of 151 tertiary comprehensive hospitals in China.Results Positive continuous improvement effects are identified in clinical laboratory system construction,personnel,reagents and calibration products management,laboratory information system,24-hour emergency testing services,and critical value report,etc.While shortcomings are also showed in management and approval of new projects,centralized and unified management of clinical laboratories,POCT project management,laboratory safety and quality control,these problems are more prominent and common,and there is still room for improvement in the clinical laboratory.Conclusion Tertiary comprehensive hospital should prioritize the pervasive problems and apply scientific management tools for systematic analysis and problem tracing so as to continuously improve clinical laboratory management.
