四肢难治性感染创面患者皮瓣移植术治疗临床疗效

摘　　要：目的探讨四肢难治性感染创面患者皮瓣移植术治疗临床疗效及对患者换药时间、疼痛缓解时间的影响。方法选取2018年1月至2020年2月我院收治的四肢难治性感染创面患者64例,随机分成两组,每组各32例,对照组进行常规的药物清创治疗,研究组行皮瓣移植术。结果与对照组(65.6%)比较,研究组的临床治疗总有效率(90.6%)明显更高(P<0.05);与对照组(3.0±0.2)d、(7.5±2.5)d、(18.9±4.5)d、(21.6±7.0)d相比较,研究组的换药时间(4.0±0.5)d、疼痛缓解时间(3.5±0.3)d和创面愈合时间(10.0±3.0)d以及住院天数(8.5±2.6)d等临床观察指标均更优(P<0.05);与对照组(18.8%)比较,研究组的皮瓣下形成腔隙、低蛋白血症和贫血等并发症发生率(3.1%)更低(P<0.05);与对照组相比较,研究组的生存质量明显更高(P<0.05)。结论应用皮瓣移植术方式治疗四肢难治性感染创面患者可以明显优化临床各项观察指标,患者的换药时间以及缩短疼痛缓解时间,降低并发症的发生率,保障患者在治疗期间的安全性,进一步提高患者的生存质量,促进治疗效果趋向于理想化,值得推广。Objective To explore the clinical effect of skin flap transplantation for patients with refractory infection of limbs and its influence on the time of dressing change and pain relief.Methods 64 patients with refractory infectious wounds of limbs treated in our hospital from January 2018 to February 2020 were selected and they were randomly divided into two groups,with 32 cases in each group.The control group was treated with conventional debridement and the study group was treated with skin flap transplantation.Results Compared with the control group(65.6%),the clinical effect of the study group(90.6%)was significantly higher(P<0.05).Compared with the control group[(4.0±0.5)d,(7.5±2.5)d,(18.9±4.5)d and(21.67.0)d],the time of dressing change,pain relief,wound healing and hospitalization stay in the study group were(3.0±0.2)d,(3.5±0.3)d,(10.0±3.0)d and(8.5±2.6)d,respectively(P<0.05).Compared with the control group(18.8%),the incidences of complications such as cavity formation,hypoproteinemia and anemia in the study group were lower(3.1%)(P<0.05).Compared with the control group,the quality of life in the study group was significantly higher(P<0.05).Conclusion Skin flap transplantation can obviously optimize clinical observation indexes,increase dressing change time,shorten pain relief time,reduce the incidence of complications,ensure the safety of patients during treatment,further improve the quality of life of patients,and promote the idealization of treatment effect,which is worth popularizing.
