沙库巴曲缬沙坦治疗终末期肾病合并射血分数降低心力衰竭1例

摘　　要：心力衰竭是终末期肾病(End-stage renal disease,ESRD)常见并发症,它严重影响患者的生存质量及预后,也给个人和社会造成极大的经济负担,故而改善ESRD患者心力衰竭显得尤为重要和迫切。慢性心力衰竭传统治疗药物一般是指血管紧张素转换酶抑制剂(Angiotensin converting enzyme inhibitor,ACEI)或血管紧张素受体阻断剂(Angiotensin receptor blocker,ARB)与β受体阻滞剂及醛固酮拮抗剂。以上三药联合称之为“金三角”,可以改善心肌重构,降低心衰患者住院及心血管死亡风险,是目前慢性心力衰竭的基本治疗方案。近年,血管紧张素受体脑啡肽酶抑制剂—沙库巴曲缬沙坦可显著改善心衰患者预后,成为治疗射血分数降低心力衰竭(Heart failure with reduced ejection fraction,HFrEF)的全新药物。目前本血液净化中心一例血液透析合并心力衰竭患者使用该药后心脏功能得到显著改善,介绍如下。Heart failure is a common complication of end-stage renal disease(ESRD),which seriously affects the quality of life and prognosis of patients,and also causes a great economic burden on individuals and society.Therefore,it is particularly important and urgent to improve heart failure in patients with ESRD.The traditional treatment drugs for chronic heart failure generally refer to angiotensin converting enzyme inhibitors(ACEI)or angiotensin receptor blocker(ARB),β-receptor blockers,and aldosterone antagonists.The combination of the above three drugs is called the"Golden Triangle",which can improve myocardial remodeling and reduce the risk of hospitalization and cardiovascular death in patients with heart failure.It is the current basic treatment for chronic heart failure.In recent years,the angiotensin receptor enkephalinase inhibitor sacubitril/valsartan can significantly improve the prognosis of patients with heart failure,and become a new drug for the treatment of heart failure with reduced ejection fraction(HFrEF).At present,a patient with hemodialysis and heart failure in our blood purification center has significantly improved heart function after using the drug,as described below.
