基于数据挖掘分析史正刚教授治疗小儿积滞的组方规律

摘　　要：目的基于数据挖掘分析史正刚教授治疗小儿积滞的组方用药规律。方法选取2018年9月至2019年9月甘肃中医药大学附属医院名老中医工作室史正刚教授儿科门诊中小儿积滞的处方共计368首,录入Excel 2016版本软件,分析处方中主要证型频数和单味中药的使用频数、频率。结果小儿积滞以脾虚夹积证最为常见,其次为痰食内积证、痰食内滞证。368首处方中以保和丸和达原饮为基础方,涉及中药93味,其中22味药物频数>100次,10味药物频数>50次,23味药物频数>10次,38味药物频数<10次。结论史教授认为小儿积滞的病机为乳食停聚、积而不化、气滞不行,临床证型以脾虚夹积证、痰食内积证和痰食内滞证为主,以消食导滞、理气化积为基本治疗原则,基础方为保和丸和达原饮。Objective To analyze composing prescription rules of professor SHI Zhenggang’s treatment of children with dyspepsia based on data mining.Methods From September 2018 to September 2019,368 prescriptions for children with dyspepsia of professor SHI Zhenggang, Outpatient Service of Pediatrics of Famous Veteran Doctor of TCM Studio in the Affiliated Hospital of Gansu University of Chinese Medicine, were chosen and entered into Excel 2016 version software to analyze the frequency of main syndrome types and the use frequency and frequency of single Chinese materia medica(CMM).Results It is most common to see dyspepsia due to asthenia of spleen syndrome, followed by phlegm and food internal accumulation syndrome as well as phlegm food internal stagnation syndrome.The 368 prescriptions were based on Baohe Wan(保和丸)and Dayuan Yin(达原饮),involving 93 flavors of CMM, of which 22 were more than 100 times, 10 were more than 50 times, 23 were more than 10 times and 38 were less than 10 times.Conclusion Professor SHI holds that the pathogenesis of children with dyspepsia is qi stagnation failure to activate due to retention of milk and food causing accumulation thereof failure to digest.The main clinical syndrome types are dyspepsia due to asthenia of spleen syndrome, phlegm and food internal accumulation syndrome as well as phlegm and food internal stagnation syndrome, taking promoting digestion to resolve food retention and regulating qi to digest the accumulation as the basic therapeutic principles with Baohe Wan and Dayuan Yin as the basic prescriptions.
