中医正骨手法闭合复位联合克氏针交叉内固定治疗儿童肱骨髁上骨折Gartland Ⅳ型16例疗效观察

摘　　要：目的观察中医正骨手法闭合复位联合克氏针交叉内固定治疗儿童肱骨髁上骨折GartlandⅣ型的疗效。方法回顾性分析福建省福州儿童医院儿童骨科2016年3月至2019年3月收治的GartlandⅣ型肱骨髁上骨折患儿16例,所有患儿均采用中医正骨手法闭合复位联合克氏针内外侧交叉固定,术后石膏托外固定。术后随访,评价肘关节功能恢复情况。结果 16例患儿中,优13例(81.25%),良2例(12.5%),可1例(6.25%),优良率为93.75%(15/16)。患儿均未出现畸形愈合、克氏针松动或断裂、针道感染、医源性血管或神经损伤、骨筋膜室综合征、肘内翻、活动受限等情况。结论中医正骨手法闭合复位联合克氏针交叉内固定治疗儿童肱骨髁上骨折GartlandⅣ型疗效确切,具有术后关节功能恢复快、损伤小且并发症少等优点。Objective To observe the clinical efficacy of TCM closed manual reduction combined with Kirschner wire internal cross fixation in the treatment of Gartland type Ⅳ of humeral supracondylar fracture in children.Methods From March 2016 to March 2019,16 children with Gartland type Ⅳ of humeral supracondylar fracture admitted into pediatric orthopedics department of Fuzhou Children’s Hospital in Fujian Province were retrospectively analyzed.All the children were treated with TCM closed manual reduction combined with medial and lateral cross Kirschner wire internal fixation, and exopexy with plaster support after operation.All patients were followed up to evaluate the functional recovery of elbow joint.Results Among the 16 cases, 13 cases were excellent(81.25%),2 cases were good(12.5%),1 case was fair(6.25%),and the excellent and good rate was 93.75%(15/16).No malunion, loosening or breaking of Kirschner wire, Kirschner wire tract infection, iatrogenic vascular or nerve injury, osteofascial compartment syndrome, cubitus varus, limitation of activity, etc.were found.Conclusion It has definite efficacy with the advantages of rapid postoperative recovery of joint function less injury, less complications to use TCM closed manual reduction combined with Kirschner wire internal cross fixation in the treatment of Gartland type Ⅳof humeral supracondylar fracture in children.
