摘　　要：目的探讨3D打印辅助髋臼金属支架在老年髋臼骨缺损翻修中的应用效果。方法采用3D打印辅助髋臼金属支架搭配骨水泥翻修治疗18例老年髋关节置换术后髋臼骨缺损患者。术前应用3D打印机按1∶1比例打印骨盆模型。股骨柄翻修:5例采用生物型股骨假体柄,7例采用骨水泥型股骨假体柄,6例保留原股骨假体柄。结果患者均获得随访,时间8~19个月。术后切口均一期愈合。末次随访时,髋臼假体位置合适,无松动及脱位,髋关节疼痛症状明显缓解或消失,髋关节Harris评分为86~93分。结论采用3D打印辅助髋臼金属支架搭配骨水泥行老年髋臼骨缺损翻修,髋臼假体可获得即刻的初始稳定,无需植骨,近期疗效满意。Objective To investigate the application effect of 3D printing assisted acetabular metal stent in the revision of acetabular bone defect in the elderly.Methods The 18 elderly patients with acetabular bone defect after hip replacement were treated with 3D printing assisted acetabular metal stent and bone cement revision.At the preoperation,3D printer was used to print the pelvis model in 1∶1 ratio.The femoral stem revision:which was performed in 5 cases with biological femoral prosthesis stem,7 cases with cemented femoral prosthesis stem,and 6 cases with original femoral prosthesis stem.Results All patients were followed up for 8~19 months.All the incisions were healed in the first stage.At the last follow-up,the acetabular prosthesis was in suitable position,without loosening or dislocation,and the pain symptoms of the hip joint were significantly relieved or disappeared.Harris hip score was 86~93 scores.Conclusions The 3D printing assisted acetabular metal stent with bone cement are used in the revision of acetabular bone defect in the elderly patients,the acetabular prosthesis can obtain immediate initial stability,which don′t need bone grafting,and the short-term effect is satisfactory.
