摘 要:目的:探讨3D打印技术在个体化髋臼翻修术中的应用。方法:收集2例应用3D打印技术的复杂髋臼翻修病例。对患者进行X线、CT检查,利用软件进行三维重建、模拟手术并设计垫片;打印聚乳酸仿真骨折模型、垫片、髋臼模型,进行模拟手术。最后打印钛合金垫片和骨小梁髋臼,进行手术。记录手术时间、出血量。术后复查X线,测量旋转中心的纵轴位置、旋转中心水平位置、股骨偏距、双下肢长度差、术后臼杯外展角、前倾角。记录术前、术后半年、1年Harris髋关节功能评分。结果:术后影像学显示缺损髋臼得到填充,髋关节假体位置良好,旋转中心和偏心距解剖重建良好,Harris评分提升显著,患者髋关节功能恢复良好。结论:3D打印技术应用于复杂髋臼翻修病例,成功重建了髋臼的旋转中心,手术疗效良好。Objective: To investigate the application of 3 D printing technology in individualized acetabular revision surgery. Methods: Two cases of complicated acetabular revision using 3 D printing technology were collected. The patients were examined by X-ray and CT, and 3 D reconstruction, simulated operation and gasket were designed by software. The polylactic acid simulation fracture model, gasket and acetabulum model were printed for the simulation operation. Finally, titanium alloy gasket and bone trabecular acetabulum were printed for operation. The operation time and blood loss were recorded. X-ray examination was performed after operation to measure the longitudinal axis position of rotation center, horizontal position of rotation center, femoral offset,length difference of lower limbs, abduction angle of acetabular cup and anteversion angle. Harris hip function score was recorded before operation, half a year and one year after operation. Results: Postoperative imaging showed that the defect of the acetabulum was filled, the hip prosthesis was in good position, the rotation center and the eccentricity were anatomically reconstructed, the Harris score improved significantly, and the patient’s hip function recovered well. Conclusion: 3 D printing technology can successfully reconstruct the rotation center of acetabulum in complex acetabular revision cases, and the surgical effect is good.
