摘　　要：目的了解聚髌器联合克氏针用于髌骨骨折内固定的临床疗效及安全性。方法纳入80例髌骨骨折为研究对象,根据内固定方法分为三组:A组行聚髌器联合克氏针内固定,30例;B组行克氏针钢丝张力带内固定,35例;C组行聚髌器内固定,15例。评价不同固定方法术后骨折愈合时间、膝关节功能及并发症。结果A组、B组及C组患者骨折愈合时间分别为(11.6±2.0)周、(11.2±2.4)周与(13.1±2.1)周(P=0.023),A组、B组显著低于C组(P<0.05),A组与B组不存在统计学差异(P>0.05)。A组、B组及C组患者术后6个月的膝关节功能优良率分别为86.7%、88.6%与66.7%(P=0.038),A组、B组显著高于C组(P<0.05)。A组、B组及C组患者术后6个月膝关节屈曲角度平均值分别为(133.2±9.4)°、(135.6±7.5)°与(123.7±10.2)°(P<0.001),A组、B组显著高于C组(P<0.05),A、B组不存在统计学差异(P>0.05)。A组患者术后疼痛、上下楼梯困难、下蹲困难的发生比例最低,显著低于B组与C组(P<0.05)。结论聚髌器联合克氏针行髌骨骨折复位内固定术可提供良好的内固定效果,同时降低术后并发症的发生风险。Objective To evaluate the efficacy and safety of patellar concentrator combined with kirschner wire for internal fixation of patella fractures.Methods Eighty patients with patella fractures was divided into group A,group B and group C according to internal fixation Methods,patients in Group A were treated with patellar concentrator combined with Kirschner wire fixation,30 cases;patients in Group B were treated with tension band wire and kirschner pin,35 cases;patients in Group C were treated with patellar concentrator alone,15 cases.Time of fracture union,the recovery of knee function and complications were compared.Results Time of fracture union in group A,group B and group C were(11.6±2.0)weeks,(11.2±2.4)weeks and(13.1±2.1)weeks(P=0.023),respectively.In addition,group A and group B showed lower incidence rate of complications than group C(P<0.05),and there was no statistical difference between group A and group B(P>0.05).The rate of patients with excellent lysholm knee scoring scale in group A,group B and group C were 86.7%、88.6%与66.7%(P=0.038),respectively.The average knee flexion angles of patients in group A,B and C were(133.2±9.4)°、(135.6±7.5)°and(123.7±10.2)°(P<0.001),respectively.The results of the pairwise comparison showed that group A and group B were significantly lower than group C(P<0.05),and there was no statistical difference between group A and group B(P>0.05).The incidence of postoperative pain,knee dysfunction in group A were the lowest,which was significantly lower than that in group B and C(P<0.05).Conclusion Using of patellar concentrator combined withkirschner wire can support a rigid internal fixation effect,whilereducing the risk of postoperative complications,which is of great value for patella fractures.
