ICU护士心理资本在创伤后应激障碍与报警疲劳中的中介作用

摘　　要：目的探讨重症监护病房(ICU)护士创伤后应激障碍对报警疲劳的影响,并在此基础上,分析心理资本在创伤后应激障碍和报警疲劳中的中介作用。方法 2019年9—11月,通过便利取样的方法,对上海市14所三级甲等综合医院1 056名ICU护士分别采用创伤后应激障碍量表平民版(PCL-C)、ICU护士报警疲劳量表、护士心理资本问卷中文版(PCQ-R)进行调查分析,并构建结构方程模型。结果不同年龄、婚姻状况、子女情况、ICU工作年限、职称的ICU护士创伤后应激障碍、报警疲劳、心理资本得分差异均有统计学意义(均P<0.05);不同学历的ICU护士创伤后应激障碍、报警疲劳得分差异均有统计学意义(均P<0.05);不同月收入的ICU护士报警疲劳得分差异有统计学意义(P<0.05)。创伤后应激障碍得分与报警疲劳得分呈正相关(P<0.05),与心理资本得分呈负相关(P<0.05);心理资本得分与报警疲劳得分呈负相关(P<0.05)。Bootstrap法检验结果显示,创伤后应激障碍到报警疲劳的直接效应、间接效应的95%CI均未包含0,心理资本的中介模型成立,所产生的部分中介效应值为0.128,占总效应的66.34%。结论 ICU护士创伤后应激障碍通过心理资本对报警疲劳起中介效应。Objective To explore the effect of post-traumatic stress disorder on alarm fatigue among nurses in intensive care unit(ICU),analyze the mediating role of psychological capital between post-traumatic stress disorder and alarm fatigue. Methods By means of convenient sampling,1 056 ICU nurses from 14 grade-A tertiary hospitals in Shanghai were investigated and analyzed using the civilian version of the post-traumatic stress disorder scale(PCL-C),the ICU nurses’ alarm fatigue scale,and the Chinese version of the nurses’ psychological capital questionnaire(PCQ-R),and the structural equation model was constructed. Results There were statistically significant differences in scores of post-traumatic stress disorder, alarm fatigue and psychological capital among ICU nurses with different age,marital status,children,working years in ICU and professional titles(all P<0.05). There were statistically significant differences in scores of post-traumatic stress disorder and alarm fatigue among ICU nurses with different educational backgrounds(all P<0.05). There was statistically significant difference in score of alarm fatigue among ICU nurses with different monthly incomes(P<0.05). The score of post-traumatic stress disorder was positively correlated with alarm fatigue score(P<0.05) and negatively correlated with psychological capital score(P <0.05). Psychological capital score was negatively correlated with alarm fatigue score(P<0.05). The results of Bootstrap test showed that the 95% CI of direct and indirect effects from post-traumatic stress disorder to alarm fatigue did not include 0. The mediating model of psychological capital was established, and the partial mediating effect value generated was 0.128, accounting for 66.34% of the total effect. Conclusion Post-traumatic stress disorder of ICU nurses has a mediating effect on alarm fatigue through psychological capital.
