摘　　要：目的通过对18家医院433例CT受检者的剂量报告统计分析,初步了解河北省CT受检者辐射剂量水平。方法选择不同区域、不同级别的医院作为调查的采样医院,通过医院CT医学影像系统随机选择2017年接受CT放射诊断的成年受检者,针对样本受检者剂量报告中显示数据进行计算建立数据库,用SPSS 18.0软件对数据进行分析。结果本次调查结果各部位容积CT剂量指数(CTDIvol)及剂量长度乘积(DLP)的中位值均满足WS/T 637—2018《X射线计算机断层摄影成年人诊断参考水平》指导水平的要求,与其他省市调查结果相比,腰、胸、腹、四肢CTDIvol、加权CT剂量指数(CTDIw)或DLP的75%分位值差异较大,多处差异超出20%。结论 CT检查实践活动应在指导水平(DRL)的约束下进行,各省应当在国家级DRL的基础上通过剂量调查制定各自的省级DRL,给出更详细的剂量指导水平规范辖区内的CT检查实践。Objective By the statistical analysis of dose reports of 433 subjects undergoing CT examination from 18 hospitals,to preliminarily understand the radiation dose levels of subjects undergoing CT examination in Hebei Province. Methods The hospitals of different levels in different regions were chosen for survey. The adult subjects undergoing CT radiological diagnosis in217 were randomly selected by the CT medical imaging system in hospital. And the data in the dose report of the subjects were calculated to build database,and SPSS 18.0 software was used in data analysis. Results The results showed that the median values of volume CT dose index(CTDIvol) and dose length product(DLP)in all sites met the requirements of the diagnostic reference level for adults in X-ray computed tomography(WS/T 637-2018). Compared with the results of other provinces and cities,the 75% percentile of CTDIvoland CTDIwor DLP in waist,chest,abdomen,limbs were significantly different,and the differences were more than 20% in many parts. Conclusion The practice of CT examination should be carried out under the constraint of DRL. And the provincial DRL should be enacted through dose survey on the basis of the national DRL,so as to give more detailed dose guidance levels to standardize CT examination practice within the jurisdiction.
