躯体-社会快感缺失在胃癌患者术后状态-特质焦虑与创伤后应激障碍间的中介效应

摘　　要：目的探讨躯体-社会快感缺失在胃癌患者术后状态-特质焦虑与创伤后应激障碍之间的中介效应,为患者术后的心理护理提供参考。方法采用便利抽样方法,应用中文版修订躯体快感缺失量表、中文版修订社会快感缺失量表、状态-特质焦虑问卷、创伤后应激障碍测评表第五版对杭州市3家医院251例胃癌术后患者进行调查研究。结果胃癌患者术后躯体快感缺失得分为(44.36±3.22)分,社会快感缺失得分为(28.62±3.27)分,状态焦虑得分为(59.12±6.25)分,特质焦虑得分为(60.35±6.34)分,创伤后应激障碍得分为(60.45±6.21)分。路径分析可得,胃癌患者术后状态焦虑对创伤后应激障碍的直接效应为0.422,躯体快感缺失中介效应为0.167,社会快感缺失的中介效应为0.189。胃癌患者术后特质焦虑对创伤后应激障碍的直接效应为0.433,躯体快感缺失的中介效应为0.193,社会快感缺失的中介效应为0.170。结论躯体-社会快感缺失在胃癌患者术后状态-特质焦虑和创伤后应激障碍之间存在中介效应作用。Objective To study the mediating effect of body-society anhedonia between postoperative state-trait anxiety and post-traumatic stress disorder(PTSD)for patients with gastric cancer to provide a reference for postoperative psychological nursing.Method Choose 251 patients with gastric cancer from 3 hospitals in Hangzhou by convenient sampling and investigate them by Chinese Version of Revised Physical Anhedonia Scale,Chinese Version of Revised Social Anhedina Scale,State-Trait Anxiety Invertory,Posttraumatic Stress Pisorder Checklist-5.Result Score on postoperative body anhedonia,society anhedonia,state anxiety,trait anxiety and PTSD are(44.36±3.22),(28.62±3.27),(59.12±6.25),(60.35±6.34)and(60.45±6.21)for patients with gastric cancer.Path analysis shows that the direct effect of postoperative state anxiety on PTSD is 0.422,mediating effect of body anhedonia and society anhedonia are 0.167 and 0.189 for patients with gastric cancer.The direct effect of postoperative trait anxiety on PTSD is 0.433,mediating effect of body anhedonia and society anhedonia are 0.193 and 0.170 for patients with gastric cancer.Conclusion Body-society anhedonia have mediating effect between postoperative state-trait anxiety and PTSD for patients with gastric cancer.
